早期序贯性营养支持治疗对慢性阻塞性肺疾病急性加重期患者血气分析指标及肺功能的影响

Influence of early sequence nutritional support therapy on blood gas analysis and pulmonary function of acute exacerbation of chronic obstructive pulmonary disease patients

目的探讨早期序贯性营养支持治疗对慢性阻塞性肺疾病急性加重期(AECOPD)患者血气分析指标及肺功能的影响。方法选取衡水市第二人民医院2014年6月—2017年6月收治的AECOPD患者68例,随机分为对照组和观察组,各34例。对照组患者给予全肠外营养治疗,观察组患者给予早期序贯性营养支持。比较两组患者治疗前后肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、呼气中段流量(MMEF)及最大呼气峰流速(PEF)]、血气分析指标[pH值、血氧分压(Pa O2)、血二氧化碳分压(Pa CO2)]。结果治疗前,两组患者FVC、FEV1、MMEF、PEF、pH值、Pa O2、Pa CO2比较,差异无统计学意义(P>0.05);治疗后,观察组患者FVC、FEV1、MMEF、PEF、pH值、Pa O2高于对照组,Pa CO2低于对照组(P<0.05)。结论早期序贯性营养支持可有效改善AECOPD患者肺功能和血气分析指标,提升治疗效果。

Objective To investigate the influence of early sequence nutritional support therapy on blood gas analysis and pulmonary function of acute exacerbation of chronic obstructive pulmonary disease（AECOPD） patients.Methods A total of 68 cases of patients with AECOPD were selected from June 2014 to June 2017 in the Second People＇s Hospital,which were randomly divided into control group and observation group,34 cases in each group.The control group was treated with total parenteral nutrition,observation group was treated with early sequence nutritional support therapy.The pulmonary function（FVC,FEV1,MMEF,PEF） and blood gas analysis indexes（pH value,Pa O2,Pa CO2） before and after treatment were compared between the two groups.Results Before treatment,no significant differences of FVC,FEV1,MMEF,PEF or pH value,Pa O2,Pa CO2 was found between the two groups（P〉0.05）;after treatment,the observation group of FVC,FEV1,MMEF,PEF,pH value,Pa O2 were higher than control group,Pa CO2 was lower than control group（P〈0.05）.Conclusion Early sequence nutritional support therapy can effectively improve the pulmonary function and blood gas analysis of AECOPD patients,improve the clinical effect.
