应用3.0 T磁共振和256层螺旋CT研究脑泡型包虫病影像特征 被引量:1

Application of 3.0 T MR and 256-slice Spiral CT in the Study of Imaging Features of Cerebral Alveolar Echinococcosis

摘要 目的描述脑泡型包虫病(cerebral alveolar echinococcosis,CAE)在3.0 T磁共振和256层螺旋CT的影像学特征,对比两种影像学检查的优势。方法回顾性分析经手术病理确诊的21例CAE患者的3.0 T磁共振和256层螺旋CT资料(16例3.0 T磁共振,18例256层螺旋CT,包括平扫和增强扫),对病灶的数量、大小、位置,内部结构和轮廓、周围水肿情况进行描述。结果 21例患者共45个病灶,病灶最大直径0.6~5.6 cm,其中12个位于顶叶,13个位于额叶,10个位于颞叶,5个位于枕叶,5个位于小脑半球。病灶呈实性、囊实性或囊性改变,病灶形态规整,边界清晰,病灶周围出现不同程度水肿。结论 CT和MRI检查是诊断CAE的重要手段,但是CT检查在钙化方面影像表现优于MRI,而MRI可以更好地显示囊肿内部情况并确定其解剖位置与邻近结构的关系,这对手术计划更有帮助。 Objective To characterize cerebral alveolar echinococcosis( CAE) with 3.0 T MR and 256-slice spiral CT,and compare the advantages of the two imaging approaches.Methods The clinical data of 3.0 T MR and 256-slice spiral CT in 21 CAE patients confirmed by pathology was retrospectively analyzed. Sixteen patients underwent 3.0 T magnetic resonance examination and 18 patients underwent 256-slice spiral CT examination,including both plain and enhanced scans. The number,size,location,internal structure and outline of lesions and peripheral edema were described.Results A total of 45 lesions were found in21 patients. The maximum lesion diameter was 0.6-5.6 cm. Of these lesions,12 were located in the parietal lobe,13 in the frontal lobe,10 in the temporal lobe,5 in the occipital lobe,and 5 in the cerebellar hemisphere. The lesions were solid,cystic or solid combined cystic changes. The shape of the lesions was regular,the boundary was clear,and there were different degrees of edema around the lesions.Conclusion CT and MR examinations are important methods for diagnosing CAE,but CT examination is superior to MRI in calcification. MRI can better show the internal conditions of cysts and determine the relationships between their anatomical locations and adjacent structures,which is more helpful for surgical planning.
