老年糖尿病患者行人工髋关节置换术的围手术期护理体会

Nursing Experience of Artificial Hip Replacement in Senile Diabetes Patients During the Perioperative Period

目的探究围手术期护理在老年糖尿病患者行人工髋关节置换术中的临床应用。方法选取该院2014年12月—2017年12月收治的134例行人工髋关节置换术的糖尿病患者作为研究对象,其中67例实施常规护理作为参照组,另67例实施围手术期护理作为研究组,观察两组患者治疗和护理效果。结果研究组在各个时间窗的疼痛评分均明显低于参照组,对比差异有统计学意义(P<0.05);两组术后血糖均有不同程度的升高,研究组空腹血糖和餐后2h血糖均明显低于参照组,差异有统计学意义(P<0.05);研究组各时间段髋关节功能评分明显高于参照组,差异有统计学意义(P<0.05);研究组不良反应发生率为2.99%,明显低于参照组的11.94%,差异有统计学意义(P<0.05)。结论对行人工髋关节置换术的老年糖尿病患者实施围手术期护理可显著提高治疗效果,促进髋关节功能的恢复,避免血糖升高,改善预后质量。

Objective To study the clinical application of artificial hip replacement in senile diabetes patients during the perioperative period. Methods 134 cases of diabetes patients with artificial hip replacement admitted and treated in our hospital from December 2014 to December 2017 were selected and divided into two groups with 67 cases in each, the control group used the artificial hip replacement, while the research group used the nursing during the perioperative period, and the treatment and nursing effect were observed. Results The pain scores at various time windows in the research group were obviously lower than those in the control group, and the differences were statistically significant （P〈0.05）, and the blood glucose of the two groups after surgery increased to a different degrees, and the fasting blood glucose and postprandial 2 h blood glucose in the research group were obviously lower than those in the control group, and the differences were statistically significant（P〈0.05）, and the hip joint function scores at various time stages in the research group were obviously higher than those in the control group, and the differences were statistically significant （P〈0.05）, and the incidence rate of adverse reactions in the research group was obviously lower than that in the control group （2.99% vs 11.94%）, and the differenee was statistically signifieant（P〈0.05）. Conclusion The implementation of nursing during the perioperative period of senile diabetes patients with artificial hip replacement can obviously improve the treatment effect, promote the recovery of hip joint function, avoid the increase of the blood glucose and improve the prognosis and quality.
