切开减压椎弓根内固定治疗急性脊柱创伤患者的临床分析

Clinical Analysis of Incision and Decompression Pedicle Internal Fixation for Patients with Acute Spinal Trauma

目的对切开减压椎弓根内固定方法治疗急性脊柱创伤的效果进行探讨。方法选取该院2014年1月—2017年6月期间收治的116例急性脊柱创伤患者进行研究,随机分为观察组与对照组,各58例,对照组给予保守治疗,观察组采用切开减压椎弓根内固定方法治疗,对两组治疗效果进行对比。结果与对照组对比,观察组椎管占位率、椎体高度(前、后)及伤椎Cobbs角分别为(4.81±2.09)%、(5.21±1.12)mm、(3.61±1.57)mm、(4.31±3.62)%,两组差异有统计学意义(t=7.162、10.174、6.097、5.413,P<0.05);治疗总有效率方面,观察组(96.55%)明显高于对照组(82.76%),差异有统计学意义(χ~2=12.016,P<0.05)。结论对急性脊柱创伤患者采用切开减压椎弓根内固定方法治疗,效果显著,对患者后续恢复具有促进作用,在临床中值得推广应用。

Objective To investigate the effect of open reduction and decompression pedicle internal fixation on acute spinal trauma. Methods A total of 116 patients with acute spinal trauma who were admitted to the hospital from January 2014 to June 2017 were randomly divided into observation group and control group, 58 cases in each group. The control group received conservative treatment and the observation group used incision decompression and pedicle screw internal fixation, to compare the therapeutic effects of the two groups. Results Compared with the control group, the occupancy rate of spinal canal, vertebral height（anterior and posterior） and injured vertebral Cobbs angle were （4.81±2.09）%, （5.21±1.12）mm, and （3.61±1.57）mm, （4.31±3.62）%, there was a significant difference between the two groups （t=7.162, 10.174, 6.097, 5.413, P〈0.05）. The total effective rate of treatment was significantly higher in the observation group（96.55%） than in the control group （82.76%）, the difference was statistically significant （χ^2=12.016, P〈0.05）. Conclusion The treatment of patients with acute spinal trauma treated with open reduction and pedicle screw internal fixation has significant effect on the subsequent recovery of patients. It is worthy of popularization and application in clinical practice.
