中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值和红细胞分布宽度对急性高甘油三酯血症性胰腺炎严重程度的预判价值

Predicting values of Neutrophil-Iymphocyte Ratio,Platelet-lymphocyte Ratio and Red Blood Volume Distribution Width for the Severity of Acute Hypertriglyceridemic Pancreatitis

目的探讨中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)和红细胞分布宽度(RDW)在急性高甘油三酯血症性胰腺炎(AHTGP)病情严重程度评估中的价值。方法回顾性分析63例AHTGP患者,根据亚特兰大新分类标准进行严重程度分级,轻症急性胰腺炎组32例,中度重症急性胰腺炎组20例,重症急性胰腺炎组11例。应用Kruskal-Wallis秩和检验比较AHTGP不同严重程度组间入院0小时和入院48小时NLR、PLR和RDW值的差别,并行Spearman相关性分析。结果三组入院0小时PLR、RDW差异无显著性(P>0.05),入院0小时NLR差异有显著性(P<0.05),入院48小时NLR、PLR、RDW差异有显著性(P<0.05)。Spearman相关性分析,入院48小时NLR、PLR、RDW和入院0小时NLR值与AHTGP严重程度呈正相关(P<0.05)。结论入院48小时NLR、PLR、RDW和入院0小时NLR值在预测AHTGP严重程度上具有一定的临床意义,可为临床提供参考。

Objective To investigate whether the results of neutrophil to lymphocyte ratio（NLR）, platelet to lymphocyte ratio（ PLR） and red cell volume distribution width（ RDW） can predict the severity of acute hypertriglyceridaemic pancreatitis（ AHTGP）. Method The clinical data of patients with AHTGP were enrolled retrospectively and divided into the mild acute pancreatitis（MAP, n=32）, moderately severe acute pancreatitis（MSAP, n=20）, and severe acute pancreatitis（SAP, n=11） groups. Kruskal-Wallis test was used to compared the difference of the results of NLR, PLR and RDW among the different severity of AHTGP groups, and the Spearman correlation analysis of was performed. Result PLR and RDW 0 h after admission had no significant difference among the three groups（P〈0.05）, NLR 0 h after admission had significant difference among the three groups（ P〉0.05）. NLR, PLR and RDW 48 h after admission had significant difference among the three groups（P〈0.05）. Spearman correlation analysis showed significantly positive correlation between the severity of AHTGP and the results of NLR, PLR and RDW 48 h after admission, and NLR 0 h after admission（P〈0.05）. Conclusion NLR, PLR, RDW 48 h after admission and NLR 0 h after admission might have certain clinical significance for predicting the severity of AHTGP,which would play a reference role in clinical application.
