萎胃汤调控慢性萎缩性胃窦炎胃窦黏膜组织Lgr5蛋白、E-cad蛋白表达的研究

Study of Weiwei Decoction in Regulating Expression of Lgr5 Protein and E-cad Protein in Gastric Antral Mucosa of Patients with Chronic Atrophic Antral Gastritis

【目的】探讨慢性萎缩性胃窦炎患者口服萎胃汤治疗前后,胃窦病变组织中Lgr5蛋白、E-cad蛋白表达的变化情况。【方法】选取60例慢性萎缩性胃窦炎患者作为治疗组,另选取60例在广东省第二中医院行电子胃镜检查的健康志愿者作为正常对照组。治疗组患者给予口服萎胃汤治疗,3个月为1个疗程,连续服用2个疗程。通过电子胃镜检查观察治疗前后患者胃黏膜镜下变化,内镜组织活检术获取胃窦黏膜组织,采用蛋白免疫印迹(Western blot)技术观察治疗前后胃窦黏膜组织Lgr5蛋白、E-cad蛋白表达的差异。【结果】(1)胃镜下观察胃窦,与正常对照组比较,慢性萎缩性胃窦炎患者治疗前胃窦黏膜呈现粗糙、水肿红白相间及花斑样充血的表现,治疗后胃窦黏膜表现接近正常对照组胃窦黏膜。(2)与正常对照组比较,治疗前治疗组Lgr5蛋白呈高表达(P<0.01),E-cad蛋白呈低表达(P<0.01);治疗后治疗组Lgr5蛋白表达水平明显下调(P<0.01),E-cad蛋白表达水平明显上调(P<0.01),并均基本接近正常对照组水平(P>0.05)。【结论】萎胃汤治疗慢性萎缩性胃窦炎具有较好疗效,其机制可能与下调胃窦病变黏膜Lgr5蛋白和上调E-cad蛋白的异常表达有关。

Objective To investigate the effect of oral use of Weiwei Decoction on the expression of Lgr5 protein and E-cad protein in the gastric antrum of chronic atrophic antral gastritis（CAAG）patients. Methods Sixty CAAG patients were enrolled into the treatment group,and 60 healthy volunteers served as blank control group. Patients in the treatment group were treated with Weiwei Decoction orally. Three months constituted one treatment course,and the treatment lasted for 2 continuous courses. Histological changes of the gastric mucosa before and after treatment were observed by electron gastroscopy. The gastric antral mucosa tissues were obtained by endoscopic biopsy, and then the expression of Lgr5 protein and E-cad protein in gastric antral mucosa before and after treatment were observed by Western blotting method. Results（1）Under electron gastroscope,the gastric antral mucosa of the treatment group before treatment was rough with intermingled edema, and had plaque-like hyperemia;after treatment,the manifestations of the gastric antral mucosa of the treatment group were similar to those of the blank control group.（2）Compared with the blank control group,Lgr5 protein expression was increased and that of E-cad protein was decreased in the treatment group before treatment（P〈0.01）;after treatment,Lgr5 protein expression was down-regulated（P〈0.01）and that of E-cad protein was up-regulated（P〈0.01）in the treatment group, and their expression levels almost arrived to those of the blank control group（P〉0.05）.Conclusion Weiwei Decoction exerts certain therapeutic effect for CAAG, and its mechanism is related to the down-regulation of Lgr5 protein expression and with the up-regulation of E-cad protein expression.
