脉血康胶囊对长期性中心静脉导管功能不良的影响

Effect of maixuekang capsule on dysfunction in tunneled cuffed hemodialysis catheters

摘要 目的观察脉血康胶囊对长期性中心静脉导管功能不良的临床疗效和应用价值。方法将26例采用长期性中心静脉导管进行血透治疗的患者随机分为对照组和治疗组,对照组给予常规肝素封管,治疗组在对照组治疗基础上给予服用脉血康胶囊冶疗。观察6个月后两组导管功能不良的发生率、首次尿激酶干预时间长短及干预次数、凝血功能及出血事件、导管相关感染发生情况。结果对照组和治疗组导管动脉端功能不良的发生率均高于静脉端,分别为3.32%和2.09%(P<0.05);各组发生导管功能不良后采用尿激酶治疗,首次尿激酶干预时间分别为(34±8.62)d和(43±12.13)d(P<0.05);治疗组尿激酶干预次数减少(P<0.05);治疗组较对照组PT、TT和APTT延长(P<0.05)。两组均无严重不良反应。结论脉血康胶囊配合常规肝素封管对预防血透长期性中心静脉导管功能不良的效果较好。 Objective To investigate the clinical efficacy and application value of Maixuekang capsule in the treatment of dysfunction in the tunneled cuffed hemodialysis catheters. Methods Twenty-eight patients with tunneled cuffed hemodialysis catheters were randomly divided into the control group and the treatment group. The control group was given conventional heparin sealing tube,and the treatment group was given Maixuekang capsule on the basis of treatment in the control group. The incidence of catheter dysfunction,the duration of first urokinase intervention and the number of interventions,agulation function and bleeding events,catheter-related infections were observed in two groups after6 months. Results The incidence of ductal dysfunction in both the control group and the treatment group was higher than that of the venous end,3. 32% and 2. 09% respectively( comparison between two groups,P〈0. 05); Urokinase was used to treat the patients with ductal dysfunction in each group,the urokinase first intervention time was respectively( 34 ± 8. 62) d and( 43 ± 12. 13) d( P〈0. 05); in the treatment group,the number of urokinase intervention decreased( P〈0. 05); PTT TT and APTT in the treatment group were longer than those in the control group( P〈0. 05). There was no serious adverse reaction in the two groups. Conclusion It is effective to prevent the tunneled cuffed hemodialysis catheters by the capsule of Maixuekang capsule combined with conventional heparin sealing.
