摘要
目的应用单一经颅多普勒超声(TCD)检查联合彩色多普勒血流显像(CDFI)技术,探讨糖尿病(DM)患者脑动脉血流动力学改变,评价其在糖尿病及脑血管病变中的临床应用价值。方法首先采用TCD检查,然后采用TCD联合CDFI技术对380例糖尿病患者大脑血流动力学变化进行检测。结果 (1)大脑血流动力学改变以脑血流速度增加(66.8%)和血流PI值升高(84.5%)为主;(2)频谱形态异常(71.6%),包括P2>PI,收-舒峰间距增大,收缩峰圆钝或高尖,并伴有血流杂音信号;(3)糖尿病患者随着年龄和发病时间的延长,其脑血流速度和频谱形态异常的发生率明显增高,并具有良好的线性关系。结论 TCD检查联合CDFI技术,能够更准确地检测脑血流各参数值,对评价糖尿病脑血管病变及其发展具有重要的临床价值。
Objective To explore the clinical value of cerebral arterial hemodynamics in diabetic patients and cerebrovascular diseases by using transcranial doppler ultrasound( TCD) examination combined with color doppler flow imaging( CDFI) technology. Methods First of all,single TCD examination was used. Then transcranial TCD and CDFI were used to detect cerebral hemodynamic changes in 380 diabetic patients. Results(1)The change of cerebral hemodynamics was mainly with the increase of cerebral blood flow velocity( 66. 8%) and the increase of blood flow PI( 84. 5%).(2)Frequency spectrum abnormalities( 71. 6%),including the P2 〉PI,the interval between the collection and the peak was increased,the peak of contraction was obtuse or high,accompanied by a murmur of blood flow.(3)With the increase of age and onset time,the incidence of abnormal cerebral blood flow velocity and frequency spectrum in diabetic patients increased significantly,and had a good linear relationship. Conclusion TCD examination combined with CDFI technology can detect more accurate parameters of brain blood flow,which has important clinical value in the evaluation of diabetic cerebrovascular lesions and their development.
