摘要
目的评价ABCB1(MDR1)基因C3435T多态性与儿童急性淋巴细胞白血病(ALL)的相关性。方法通过计算机检索中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、中文科技期刊全文数据库(VIP)、万方数据库(Wanfang)、The Cochrane library、PubMed、EmBase等数据库中检索ABCB1基因C3435T多态性与儿童急性淋巴细胞白血病发生的相关文献,纳入符合标准的病例-对照研究,对其进行文献质量评估和数据的提取。以OR及其95%CI为合并效应量,用Rev Man5.0软件进行Meta分析,同时用Stata12.0软件进行敏感分析和发表偏倚评估。结果纳入符合标准的相关文献12篇,共计ALL儿童1811例,对照组儿童1856例。Meta分析结果显示,C3435T位点多态性在隐性模型、共显性模型、超显性模型下整体效应差异有统计学意义(P<0.05):隐性模型[OR=1.57,95%CI(1.06,2.32),P=0.02]、共显性模型[OR=1.47,95%CI(1.02,2.12),P=0.04)]及超显性模型[OR=0.54,95%CI(0.33,0.87),P=0.01]。按人群亚组分析结果显示,高加索人群ABCB1基因C3435T多态性与儿童急性淋巴细胞白血病的相关性差异均无统计学意义(P>0.05);而亚洲人群ABCB1基因C3435T位点多态性与儿童急性淋巴细胞白血病的相关性在隐性基因模型和共显性模型下整体效应差异有统计学意义:隐性基因模型[OR=2.25,95%CI(1.11,4.55),P=0.02],共显性基因模型[OR=2.03,95%CI(1.04,3.95),P=0.04]。结论亚洲人群中ABCB1基因C3435T多态性与儿童急性淋巴细胞白血病具有相关性,但在高加索人群中却无相关性。
Objective To evaluate the association between ABCB1( MDR1) gene C3435 T polymorphism and the acute lymphoblastic leukemia( ALL) in children. Methods The China Biology Medicine( CBM),China National Knowledge Infrastructure( CNKI),VIP,Wanfang,The Cochrane library,Pub Med,Em Base and other databases were searched to collect the articles on ABCB1 gene C3435 T polymorphism and the risk of ALL in children,included in the standard case-control study,its literature quality assessment and data extraction odds ratio( OR) and 95% confidence interval( CI) were used to determine the effect size. RevMan5. 0 software was used for the Meta analysis,publication bias evaluation and sensitivity analysis were also performed by Stata12. 0. Results Twelve articles were included in the research,1811 cases of ALL children and 1856 children in control group. Meta-analysis showed that the overall effect of C3435 T polymorphism in recessive model, co-dominant model and hyper-dominant model was statistically significant( P〈0. 05) : recessive model[OR = 1. 57,95% CI( 1. 06,2. 32),P = 0. 02],co-dominant model [OR = 1. 47,95%CI( 1. 02,2. 12),P = 0. 04) ] and hyper-dominant model[OR = 0. 54,95% CI( 0. 33,0. 87),P = 0. 01]. There was no significant difference in the C3435 T polymorphism of ABCB1 gene between the Caucasian population and the incidence of acute lymphoblastic leukemia in children( P〉0. 05). The correlation between ABCB1 gene C3435 T polymorphism and childhood acute lymphoblastic leukemia in Asian population was significantly different between recessive gene model and co-dominant model: Invisible gene model[OR = 2. 25,95% CI( 1. 11,4. 55),P = 0. 02],co-dominant gene model [OR = 2. 03,95% CI( 1. 04,3. 95),P = 0. 04]. Conclusion ABCB1 gene C3435 T polymorphism in Asian population is associated with an incidence of acute lymphoblastic leukemia in children,but has no correlation in the Caucasian population.
