腺样体切除对儿童慢性鼻窦炎疗效分析

Analysis of the effect of adenoidectomy on chronic sinusitis in children

目的探讨腺样体切除术在儿童慢性鼻-鼻窦炎(CRS)治疗的临床意义。方法对我院在2015年6月~2017年1月间收治的84例腺样体肥大伴有慢性鼻窦炎的患者进行分组,随机分为对照组(44例),观察组(40例),对照组给予药物治疗+鼻腔冲洗疗程12周,观察组实施腺样体切除术(28例),腺样体+扁桃体切除术(12例),术后联合药物治疗,治疗后1、3、6、12月评价疗效。结果治疗12月后对照组总有效率79.5%,观察组总有效率95.0%,2组间疗效差异有统计学意义(P<0.05),2组间治疗1月后的显效率有统计学差异(P<0.05);2组间3、6、12月3个时期鼻内镜评分差异有统计学意义(P<0.05)。结论 2种治疗方式对于儿童CRS均有疗效;腺样体切除术或和扁桃体切除术+药物治疗对儿童慢性鼻窦炎的治疗优于单独的药物治疗,是儿童慢性鼻窦炎伴有腺样体肥大或扁桃体肥大值得推荐的治疗方法。

Objective To explore the clinical significance of adenoidectomy in children with chronic rhinosinusitis（CRS）. Methods A total of 84 patients with adenoid hypertrophy and chronic sinusitis admitted to our hospital from June 2015 to January 2017 were randomly divided into control group（44 cases）and observation group（40 cases）. The control group was given drug treatment＋nasal irrigation for 12 weeks. The ob servation group was performed adenoidectomy（28 cases）,adenoid＋tonsillectomy（12 cases）,postoperative co mbined drug therapy. When the treatment had been finished,efficacy of the treatment was evaluated after 1,3,6,12 months. Results The total effective rate in the control group was 79.5% after 12 months of treatment,and the total effective rate in the observation group was 95.0%. The difference in efficacy between the two groups was statistically significant（P〈0.05）. The effective rate was statistically significant（P〈0.05）between the 2 groups after 1 month of treatment. There was a statistically significant difference in nasal endoscopic scores between the 3 groups during 3,6 and 12 months（P〈0.05）. Conclusions The two treatment methods are effective for children with CRS;adenoidectomy or tonsillectomy＋drug treatment is better than single drug treatment for children with chronic sinusitis,chronic sinusitis in children with adenoids Hypertrophy or tonsil hypertrophy is worthy of recommodation.
