2016年兰州市中西医临床医生职场孤独感的影响因素 被引量:3

Influencing factors of workplace loneliness among clinicians of traditional Chinese medicine and western medicine in Lanzhou City in 2016

摘要 目的了解中医和西医临床医生职场孤独感影响因素的异同。方法以职场孤独感量表、职业生活问卷、院内医院内团队组织气氛问卷、医生版医患关系问卷和中医文化认同问卷等为评定工具,对2016年兰州市3所三甲医院中320名医生进行评估。采用相关和回归等统计方法进行数据分析。结果同情心满足和组织气氛与职场孤独感呈显著负相关(-0.522≤r≤-0.264,P<0.01);工作倦怠和医患关系与职场孤独感呈显著正相关(0.270≤r≤0.520,P<0.01);中医临床医生的职场孤独感与中医文化认同呈显著负相关(r=-0.344,P<0.01);科室领导与沟通、工作协调与前景对2组医生的职场孤独感均具有解释作用(β=-0.242、0.278、-0.230、0.144,均P<0.05);同情心满足和中医文化认同对中医临床医生的职场孤独感具有解释作用(β=-0.295、-0.218,均P<0.05);工作倦怠、医患理解与交流对西医临床医生的职场孤独感具有解释作用(β=0.300、-0.301,均P<0.05)。结论影响中医和西医临床医生职场孤独感的因素有共同点也有差异性,其中组织气氛对2组医生职场孤独感影响作用具有共同点;同情心满足和中医文化对中医临床医生的职场孤独感具有独特的影响作用;而工作倦怠对西医临床医生的职场孤独感具有独特的影响作用。 [Objective]To explore the similarities and differences on influencing factors of loneliness in the workplace between the clinicians of traditional Chinese medicine and western medicine. [Methods] In 2016,320 clinicians from three grade-A tertiary hospitals in Lanzhou City were evaluated by the Loneliness at Work Scale,Professional Quality of Life-Revision IV Questionnaire,team-in-hospital organization climate scale,Difficult Doctor-Patient Relationship Questionnaire-10 and cultural identity of traditional Chinese medicine questionnaire. The data was analyzed by using the methods of correlation analysis and regression analysis. [Results] Loneliness in the workplace was significantly negatively correlated with compassion satisfaction and organization climate(-0.522 ≤r ≤-0.264,P〈0.01),and it was significantly positively correlated with job burnout and doctorpatient relationship(0.270≤r≤0.520,P〈0.01). Loneliness in the workplace of the clinicians of traditional Chinese medicine was significantly negatively correlated with the cultural identity of traditional Chinese medicine(r=-0.344,P〈0.01). Loneliness in the workplace of the clinicians in two groups was explained by team leading and communication,and position cooperation and outlook(β =-0.242、0.278、-0.230、0.144, all P〈0.05). Loneliness in the workplace of the clinicians of traditional Chinese medicine was explained by Compassion satisfaction and cultural identity of traditional Chinese medicine(β=-0.295、-0.218,all P〈0.05).Loneliness in the workplace of the clinicians of western medicine was explained by job burnout and "patience-staff comprehension and communication(β=0.300、-0.301,all P〈0.05). [Conclusion] There are similarities and differences in influencing factors of workplace loneliness between the clinicians of traditional Chinese medicine and western medicine. The organizational climate has similar effect on the workplace loneliness of two groups. Compassion satisfaction and cultural identity of traditional Chinese medicine have a unique effect on the clinicians of traditional Chinese medicine,while job burnout has a unique effect on the clinicians of western medicine.
