单侧耳软骨仿生支架在短鼻延长中的应用

Application of a biomimetic scaffold of unilateral auricular cartilage in short nose lengthening

目的介绍一种单侧耳软骨仿生支架在短鼻延长中的应用方法。方法耳后切取单侧耳软骨,制作2片对称的耳软骨块,垂直夹持鼻中隔软骨前角重叠缝合,以加强鼻中隔软骨前角,延伸鼻中隔软骨尾侧的高度和长度;斜向前上提紧鼻中隔膜部,将膜部黏膜与支架缝合固定。支架仍位于鼻翼软骨穹窿的下面,形如生理的鼻中隔软骨与鼻翼软骨位置关系解剖形态。结果本组151例短鼻延长效果确切。术后随访6~24个月,除1例鼻尖延长抬高效果较夸张,6个月后给予调整修复外,其余患者均对术后效果满意。结论单侧耳软骨仿生支架实现短鼻延长的方法简单实用,效果显著,值得临床推广应用。

Objective To introduce the application of a biomimetic scaffold of a single section of ear cartilage in short nasal extension.Methods The unilateral auricular cartilage was incised from the posterior side of the auricle,and two pieces of symmetrical ear cartilage were made to vertically clamp the anterior horn of the nasal septum cartilage.The anterior horn of the nasal septum cartilage was strengthened so that the height and length of the caudal side of the nasal septal cartilage would be extended.The membrane of the nasal septum was lifted upward obliquely and the mucous membrane was sutured with the scaffold.The scaffold was still located under the alar cartilage fornix,and the anatomical form was similar to the physiological position of the nasal septum cartilage and the nasal alar cartilage.Results Treatment in all 151 patients with short nose lengthening had good results.During the follow-up from 6 to 24 months,all patients were satisfied with the surgical results,except for one patient who felt the lengthened and raised effect of the nasal tip was exaggerated.The exaggerated nasal tip was adjusted and repaired after 6 months.Conclusion The application of a biomimetic scaffold of unilateral auricular cartilage was simple,economical,and effective for short nasal extension.It is worth popularizing in clinic.
