某院门诊药房退药特征分析及应对措施探讨

Analysis of drug withdrawal characteristics and countermeasures in outpatient pharmacy in hospital

目的探讨某院门诊药房退药特征分析及应对措施探讨。方法选择2016年1月～2017年12月某院门诊退药60例为观察对象,采用回顾性分析方法,找出退药处方中重要信息,如退药种类构成、科室发布、退药构成因素等,进行数据录入、汇总及分析。结果退药种类依次为抗生素22例(36.67%)、中成药18例(30.00%)、电解质及酸碱平衡药及维生素类10例(16.67%)等。退药科室口腔颌面外科最多16例(26.67%)、内科系统类用药14例(23.33%)、口腔科15例(25.00%)、外科10例(16.67%)、其他5例(8.33%)。退药原因包括:医师因素30张(50.00%);药房因素20张(33.33%)。结论门诊药房存在一定比例退药现象,退药种类构成、科室发布、退药构成因素复杂,应该实施针对性应对措施最大限度降低门诊药房的退药现象的发生。

Objective To diseuss the eharaeteristies of drug withdrawal in outpatient pharmaey in our hospital and to diseuss the eountermeasures. Methods From Janury 2016 to Deeember 2017 in our hospital, 60 eases of out-patient drug withdrawal were seleeted as the objeets of observation, using retrospeetive analysis, to find out the important information of preseription drug withdrawal, sueh as eomposition, types of drug withdrawal, withdrawal departments released eomponents. Results The type of drug withdrawah 22 eases of antibioties （36.67%）, Chinese patent medieine （ 18 eases） （30.00%）, Eleetrolytes and add-base balanee drugs and vitamins 10 eases （16.67%） and so on. There were 16 eases （26.67%） of drug withdrawal in the department of oral and maxillofaeial surgery, 14 eases （23.33 %） in the department of Internal Medieine, 15 eases （25.00%） in Dental Department, 10 eases （ 16.67% ） in the department of surgery, The other 5 eases （8.33%） .The reasons for the drug withdrawal inelude： 30 medieal factors （50%）, The pharmacy faetors 20 （33.33%）. Conclusion There is a eertain proportion of drug withdrawal in outpatient pharmacy. The composition of drug withdrawal, the release of depamnents and the composition factors of drug withdrawal are complex. Therefore, targeted measures should be taken to minimize the oeeun＇enee of drug withdrawal in outpatient pharmaey.
