DCD供肾肾移植术后BK病毒感染情况及危险因素分析

Analysis of BK virus infection and risk factors after renal transplantation by DCD donor kidney

目的:探讨中国心脏死亡官器捐献(DCD)供肾肾移植术后患者发生BK病毒感染的严重度及主要危险因素,为预防及治疗提供相应的依据。方法:收治肾移植术患者280例,分为BK病毒DNA阳性组(70例)及BK病毒DNA阴性组(210例)。分析DCD供肾肾移植术后患者感染发生率、感染部位及影响感染的因素。结果:其尿BK病毒DNA阳性率高于血BK病毒DNA阴性组。肺部感染、急性排斥反应与FK506和BK病毒感染密切相关。结论:DCD肾移植患者BK病毒感染的发病率明显增高,且风险降低。应加大力度控制有关危险因素,为了减少肾移植术后BK病毒感染的危险,必须严格控制基于FK506的肺部感染、急性排斥反应和免疫抑制剂。

Objective：To explore the severity and main risk factors of BK virus infection in patients after renal transplantation by DCD donor kidney,to provide the basis for prevention and treatment.Methods：280 patients with renal transplantation were selected,they were divided into the BK virus DNA positive group and the BK virus DNA negative group,we analyzed the incidence of infection,the location of infection and the factors affecting infection.Results：The positive rate of DNA in urine BK virus was higher than that of blood BK virus DNA.Pulmonary infection and acute rejection were closely related to FK506 and BK virus infection.Conclusion：The incidence of BK virus infection in DCD renal transplant recipients was significantly higher and the risk was lower.We need to strengthen the control of risk factors,in order to reduce the risk of BK virus infection after renal transplantation,we must strictly control FK506 based lung infection,acute rejection and immunosuppressive agents.
