以腹痛为首发症状的围青春期男性睾丸扭转的治疗效果

Treatment effect of testicular torsion in pubertal men with abdominal pain as the first symptom

目的分析以腹痛为首发症状的围青春期男性睾丸扭转的治疗效果。方法选择2010年1月至2017年6月我院收治的围青春期男性睾丸扭转患者25例,首发症状均为腹痛。急诊彩超检查后经手术治疗,分析治疗效果。结果男性睾丸扭转患者25例中误诊18例,其中一侧睾丸坏死、对侧睾丸固定者10例,另7例手术治疗痊愈出院;睾丸切除患者发病至手术间隔时间显著长于睾丸保留的患者(P<0.05);术后睾丸萎缩1例、恶心呕吐2例,皮肤红肿1例,皮肤紫黑12例。结论以腹痛为首发症状的围青春期男性睾丸扭转需尽早明确诊断,并给予相应的治疗措施,有助于患者早日康复。

Objective To analyze the clinical effect of testicular torsion in pubertal men with abdominal pain as the firstsymptom. Methods Twenty-five male patients with testicular torsion were treated in our hospital from January 2010 to June2017 were selected. The first symptom were abdominal pain. After emergency color doppler ultrasound examination, theywere treated with surgery. The therapeutic effects were analyzed. Results Among the 25 patients with testicular torsion, 18cases were misdiagnosed, of which 10 cases were unilateral testicular necrosis and contralateral testicular fixation. The other7 cases were cured and discharged. Patients with orchiectomy had significantly longer onset to operative time than those withtesticular preservation （P〈0.05）. There were 1 case of testicular atrophy, 2 cases of nausea and vomiting, 1 case of skinredness and 12 cases of skin purple black. Conclusion The testicular torsion in perimenopausal men with abdominal pain asthe first symptom must diagnosis as soon as possible, and give corresponding treatment measures to help early recovery.
