胃穿孔修补术采用尼龙绳及双臂钳治疗的临床效果及复发性

Clinical efficacy and recurrence of gastric perforation repair with nylon cord and double-arm forceps

目的探讨胃穿孔修补术采用尼龙绳及双臂钳治疗的临床效果及复发性。方法选取我院收治的42例急性胃穿孔患者,根据手术方式将其分为观察组(21例,给予用尼龙绳及双臂钳的胃穿孔修补术)与对照组(21例,给予胃部分切除术)。比较两组的治疗效果、临床手术指标、不良事件发生情况及复发情况。结果两组Visick分级差异显著(P<0.05);观察组治疗有效率高于对照组(P<0.05)。观察组的手术时间、首次下床活动时间、胃肠功能恢复时间、住院时间均短于对照组,术中出血量少于对照组(P<0.05)。两组不良事件总发生率比较,差异无统计学意义(P>0.05)。观察组的复发率高于对照组(P<0.05)。结论胃穿孔修补术采用尼龙绳及双臂钳治疗效果显著,但有较高复发率,应根据患者实际情况合理选择手术方式。

Objective To investigate the clinical effect and recurrence of gastric perforation repair with nylon rope anddouble-arm forceps. Methods Forty-two patients with acute gastric perforation were selected and divided into observationgroup （21 cases, treated with gastric perforation repair with nylon rope and double-arm forceps） and control group （21cases, treated with partial gastrectomy）. The therapeutic effects, clinical operation indexes, adverse events and recurrenceswere compared between the two groups. Results There was significant difference in Visick grading between the two groups（P〈0.05）. The effective rate of the observation group was higher than that of the control group（P〈0.05）. The operation time,first ambulation time, gastrointestinal function recovery time and hospitalization time of the observation group were shorterthan those of the control group, and the intraoperative bleeding volume was less than that of the control group（P〈0.05）.There was no significant difference in the total incidence of adverse events between the two groups （P〉0.05）. Therecurrence rate of the observation group was higher than that of the control group （P〈0.05）. Conclusion The repair ofgastric perforation with nylon rope and double-arm forceps has significantly effect, but there is a high recurrence rate. The operation mode should be selected according to the actual situation of the patients.
