小柴胡汤在耳鸣治疗中的应用体会

The application and experience of Xiao Chai Hu decoction in the treatment of tinnitus

长期慢性的耳鸣为耳鼻咽喉科常见病症,困扰着各个年龄段的患者,临床治疗是一大难题,中医药治疗有其独到之处.临证之时需辨明引起耳鸣的中医病因病机,运用中医整体观念、辨证施治,方能取得较好疗效.临床观察发现,肝郁气滞、肝脾不调是耳鸣发生的重要病因病机.小柴胡汤为张仲景？伤寒论？中的名方,其功效为“和解少阳、疏肝健脾”,不仅广泛应用于临床各科疾病的治疗,其治则也符合部分耳鸣的病因病机.临床实践中,运用小柴胡汤加减,或小柴胡汤与其它经方或时方合方加减,治疗耳鸣一病,常可取得较好的疗效.

Tinnitus is a common symptom in the department of E. N. T. , whose clinical treatment is considered a long standing difficulty. However,Chinese medicine treatments can bring unique effects to this. During a clinical treatment of tinnitus, etiology and pathology must be clarified in a holistic perspective of traditional Chinese medicine and through theories of dialectical treatments, so that a better efficacy can be achieved. Clinical observations discovered that important etiologies of tinnitus include stagnation of liver Qi, and disorders between liver and spleen. Xiao Chaihu Tang is a noted prescription in Treatise on Exogenous Febrile Disease ,whose effects include lifting up Yang Oi,relieving Shaoyang disorder,dispersing stagnated liver Qi,and replenishing Qi to invigorate the spleen. Not only it is widely applied in treatments of diseases in different departments,its principles also meet the etiology and pathology of some tinnitus cases. In clinical practices,the usage of adjusted Xiao Chaihu Tang,as well as combinations of Xiao Chaihu Tang with other classical or current prescriptions is proved to have some outstanding efficacies for relieving tinnitus.
