宝石能谱CT心脏高清与非高清扫描方式对冠状动脉支架显示清晰度的影响 被引量:9

摘要 目的探究宝石能谱CT心脏高清与非高清扫描模式显示冠状动脉支架的差异及对支架内腔显示的影响。方法行宝石能谱CT冠状动脉造影检查的84例冠状动脉支架植入术后患者为观察对象。根据扫描模式分为高清组(43例,支架77枚)、非高清组(41例,支架74枚)。观察比较两组主观图像质量评分、对比噪声比(CNR)及不同支架内径的支架内CT值、支架内腔显示率。结果高清组图像主观评分、CNR及支架内腔显示率分别(4. 82±1. 05)分、21. 46±3. 95、(57. 85±7. 16)%,显著高于非高清组(3. 85±0. 84)分、18. 62±3. 61、(48. 28±6. 27)%,差异有统计学意义(t=6. 253、4. 606、8. 724,P <0. 05)。对内径<2. 00mm支架,高清组支架内CT值(263. 85±37. 51) HU,高于非高清组(192. 37±26. 59) HU(t=3. 556,P <0. 05),两组支架内腔显示率无统计学差异(P> 0. 05);对内径2. 00~4. 00mm支架,高清组支架内CT值、支架内腔显示率为(366. 25±48. 17) HU、(57. 22±7. 64)%,高于非高清组(278. 54±47. 23) HU、(40. 54±6. 08)%,差异有统计学意义(t=9. 943、13. 053,P <0. 05);而对内径> 4. 00mm的支架,两组两项指标比较差异无统计学意义(P> 0. 05)。结论宝石能谱CT心脏高清扫描模式显示支架效果理想,尤其对内径2. 00~4. 00mm支架的显示效果优于非高清扫描。 Objective To explore the differences between stents in high-resolution and non-high-definition CT scans of the heart gemstone spectrum and the effect on stent lumen display. Methods Eighty-four patients undergoing coronary artery stenting with gemstone CT coronary angiography from December 2015 to September 2017 were selected as the study subjects. We divided them into high-definition group according to scan mode (43 cases, 77 stents), non-HD group (41 cases, 74 stents). We observed and compared the subjective image quality scores, contrast-to-noise ratio (CNR) , in-stent CT values of different stent diameters, and stent lumen display rates between the two groups. Results Subjective scores, CNR, and stent lumen display rates in the HD group were (4.82 ± 1.05 ) points, 21.46 ± 3.95, (57.85 ± 7.16) %, respectively, significantly higher than those in the non-HD group (3.85 ± 0.84) points and 18.62± 3.61, (48.28 ± 6.27)%, the difference was statistically significant ( t = 6. 253, 4. 606, 8. 724, P 〈 0.05). For the stents with ID 〈 2.00 ram, the CT value of the HD group was (263.85 ± 37.51 ) HU, higher than that of the non-HD group ( 192.37 ± 26.59) HU ( t = 3. 556, P 〈 0.05). There was no significant difference in the rate of lumen display between the two groups ( P 〉 0.05 ). For stents with an internal diameter of 2.00 - 4.00 mm, the CT value and stent lumen display rate in the HD group were (366.25 ±48.17) HU, (57.22 ±7.64) % higher than that in the non- HD group (278.54 ± 47.23 ) HU, (40.54 ± ) 6.08) %, the difference was statistically significant ( t = 9. 943, 13. 053, P 〈 0. 05 ) ; For stents with an internal diameter 〉 4.00 ram, there was no significant difference between the two groups ( P 〉 0. 05). Conclusion The gem energy spectrum CT-cardiac high-definition scanning mode shows an ideal stent, especially for the inner diameter of 2.00 - 4.00 mm stent display better than non-high-definition scan.
