亚低温疗法联合参附注射液治疗心脏骤停后脑复苏患者的疗效

Effect of Mild Hypothermia Combined with Shenfu Injection on Cerebral Resuscitation after Cardiac Arrest

目的探讨亚低温疗法联合参附注射液治疗心脏骤停后脑复苏患者的临床疗效。方法选取84例心脏骤停后脑复苏患者,随机分为2组,对照组42例实施亚低温疗法,观察组42例实施亚低温疗法联合参附注射液治疗,于治疗前后行临床指标(心率、呼吸频率、平均动脉压、血氧饱和度)检测和相关功能[格拉斯哥昏迷量表(GCS)评分、格拉斯哥预后量表(GOS)评分、美国国立卫生研究院卒中量表(NIHSS)评分、急性生理与慢性健康评分(APACHEⅡ)]评定,比较2组患者的治疗效果。结果与治疗前比较,治疗后2组患者心率、呼吸频率、平均动脉压、血氧饱和度、GCS评分、GOS评分增加,而NIHSS评分、APACHEⅡ评分降低(均P<0.01)。治疗后观察组患者心率、呼吸频率、平均动脉压、血氧饱和度、GCS评分、GOS评分、有效率、存活率高于对照组,而NIHSS评分、APACHEⅡ评分低于对照组(P<0.05或P<0.01)。结论亚低温疗法联合参附注射液治疗心脏骤停后脑复苏患者的疗效显著,可明显改善患者的临床指征与相关功能。

Objective To investigate the clinical effect of mild hypothermia combined with Shenfu injection on cerebral resuscitation after cardiac arrest. Methods Eighty four patients re suscitated from cardiac arrest were randomly treated with mild hypothermia alone（control group, n 42） or in combination with Shenfu injection（observation group,n 42）.Heart rate,respiratory rate, mean arterial pressure and blood oxygen saturation were detected before and after treatment. Furthermore,the related functions were evaluated using Glasgow Coma Score（GCS）, Glasgow Outcome Scale（GOS）,National institute of Health Stroke Scale（NIHSS） and Acute Physiology and Chronic Health Evaluation Ⅱ （APACHE Ⅱ ）.Therapeutic efficacy was compared between the two groups.Results After treatment, heart rate, respiratory rate, mean arterial pressure, blood oxygen saturation,OCS score and GOS score increased but NIHSS score and APACHE Ⅱ score decreased in both groups （P 〈0.01）. Compared with control group, heart rate, respiratory rate, mean arterial pressure, blood oxygen saturation, GCS score, GOS score, effective rate and survival rate increased but NIHSS score and APACHE Ⅱ score decreased in observation group after treatment（P〈0.05 or P〈0.01）.Conclusion Mild hypothermia combined with Shenfu injection can effectively improve clinical indications and related functions in patients resuscitated from cardiac arrest.
