血清TPOAb、TGAb和补体C3、C4与桥本甲状腺炎的关系

Relationships of Serum TPOAb,TGAb,C3 and C4 Levels to Hashimoto's Thyroiditis

目的探讨血清抗甲状腺过氧化物酶抗体(TPOAb)、抗甲状腺球蛋白抗体(TGAb)和补体C3、C4与桥本甲状腺炎(HT)的关系。方法将2015年8月至2017年4月接诊的HT患者60纳入观察组(n=60),其中甲状腺功能减退29例(甲减亚组),甲状腺功能亢进3例(甲亢亚组)和甲状腺功能正常28例(甲状腺功能正常亚组);选取同时期行健康体检的志愿者60例为对照组。比较观察组和对照组TGAb、TPOAb、甲状腺刺激抗体(TSAb)、补体C3和C4水平,比较观察组各亚组血清游离甲状腺素(FT4)、血清游离三碘甲状腺原氨酸(FT3)、促甲状腺激素(TSH)、补体C3和C4水平。结果观察组TGAb、TPOAb、TSAb水平及阳性率均高于对照组(P<0.05),补体C3、C4水平均低于对照组(P<0.05)。FT4、FT3水平:甲减亚组<甲状腺功能正常亚组<甲亢亚组(P<0.05);TSH水平:甲减亚组>甲状腺功能正常亚组>甲亢亚组(P<0.05);补体C3、C4水平甲减亚组明显低于甲状腺功能正常亚组和甲亢亚组(P<0.05)。结论 HT患者血清TPOAb、TGAb水平较高,补体C3、C4水平较低,而且甲减患者C3、C4水平最低,可通过以上指标的检测辅助临床治疗。

Objective To explore the relationships of serum thyroid peroxidase antibody（TPO Ab）, thyroglobulin antibody（TGAb） and complement（C3, C4） levels to Hashimoto＇s thyroiditis （HT）.Methods Sixty HT patients visiting hospital between August 2015 and April 2017 were enrolled as the observation group,including 29 cases of hypothyroidism, 3 cases of hyperthyroid ism and 28 cases of euthyroidism.At the same time,B0 healthy subjects were selected as the control group.Serum levels of TOAb,TPOAb,thyroid stimulating antibody（TSAb）, C3 and C4 were compared between observation group and control group.Furthermore, serum free triiodothyronine （FT3） and thyrotropin （TSH） levels were measured in observation group. Results Compared with control group, serum TGAb, TPOAb and TSAb concentrations increased and complement C3 and C4 levels decreased in observation group（P 〈0.05）.Compared with euthyroidism subgroup,levels of FT4 and FT3 elevated in hyperthyroidism subgroup but reduced in hypothyroidism subgroup,and levels of TSH increased in hypothyroidism subgroup but decreased in hyper thyroidism subgroup（P〈0.05）.In addition,complement C3 and C4 levels in hypothyroidism sub group were lower than those in both euthyroidism and hyperthyroidism subgroups （P 〈0.05）.Conclusion HT patients have high levels of tients with hypothyroidism have the lowest can be used to assist clinical treatment.
