脊柱骨盆矢状位参数与退变性腰椎滑脱的关系研究

Relationship between Spinal and Pelvic Sagittal Parameters and Degenerative Lumbar Spondylolisthesis

目的探讨脊柱骨盆矢状位参数特征与退变性腰椎滑脱(degenerative lumber spondylolisthesis,DLS)的关系。方法回顾性分析2015年1—12月72例DLS患者(DLS组)及健康体检的志愿者60例(对照组)的影像学资料(全脊柱侧位X线片),并测量脊柱-骨盆矢状面平衡参数,包括腰椎前凸角(1umbar lordosis,LL)、胸椎后凸角(thoracic kyphosis,TK)、骶骨倾斜角(sacral slope,SS)、骨盆入射角(pelvic incidence,PI)、骨盆倾斜角(pelvic tilt,PT)、矢状面轴向垂直距离(sagittal vertical axis,SVA)。结果 DLS组LL、TK、PI和SS分别为(46.760 0±0.721 2)°、(43.080 0±0.560 1)°、(52.850±1.026)°和(39.550±1.090)°,均明显大于对照组(P<0.01);PT、SVA分别为(24.880 0±0.596 8)°、(24.870 0±0.658 0)mm,与对照组相比差异无统计学意义(均P>0.05)。结论脊柱-骨盆矢状位平衡参数在DLS患者中有明显的差异。LL、TK、SS、PI值可能与DLS有关系。

Objective To investigate the relationship between spinal and pelvic sagittal parame ters and degenerative lumbar spondylolisthesis（DLS）.Methods Imaging data（lateral spine radio graphs） collected from 72 DLS patients（DLS group） and 60 healthy subjects（control group） be tween January and December 2015 were analyzed retrospectively. The sagittal parameters were measured, including lumbar lordosis（LL）, thoracic kyphosis（TK）, pelvic incidence（PI）, pelvic tilt （PT） ,sacral slope（SS） and sagittal vertical axis（SVA）.Results The TK,PI,SS and LL in DLS group wererespectively（43.080 0±0.560 l） °,（52.850±1.026） °,（39.550±1.090）° and （46.760 0± 0.721 2）°,which were significant higher than those in control group（P〈0.01）.The PT and SVA in DLS group were respectively（24.880 0±0.596 8）° and（24.870 0±0.658 0）ram,which differed sta tistically from those in control group（P〉0.05）.Conclusion There are significant alterations in spinal and pelvic sagittal parameters in DLS patients. The LL, TK, SS and PI may be associated with DLS.
