摘要
卒中后抑郁(PSD)是一种卒中导致的心境障碍,是卒中后最常见的神经精神症状之一,PSD显著增加卒中后的残疾率和病死率,尽早识别和治疗PSD对于改善卒中患者预后具有十分重要的临床意义。PSD相关危险因素及病因机制的研究取得重要进展,为PSD的治疗提供了新思路。文章简述了PSD的识别、危险因素、病因机制及治疗现状。认为不同致病因素在PSD的某些症状中可能发挥更显著的作用,而且其影响可能在卒中后的不同时间而有所差异。未来的研究应该尝试探索PSD特定症状的机制进而带来PSD特异性靶向治疗。
Post stroke Depression（PSD）,a mood disorder caused by stroke, is one of the most common neuropsychiatric outcomes of stroke. It leads to greater disability as well as increased mortality.Early identification and treatment of PSD contributes to the improvement of prognosis. The important advances have been made in the related risk factors and pathogenesis of PSD, which bring new thoughts on the treatment of PSD. This review presents the identification,risk factors, etiological mechanism and current treatment status of PSD.We believe that different etio logical factors may play more salient roles in some symptoms of PSD and their effects may vary at different times after stroke.Future studies should attempt to identify the mechanisms of specific symptoms of PSD for performing specific targeted treatments.
