日间手术中心运行对住院医疗费用的影响分析

Analysis of Impact of Operation of Day Surgery Center on Inpatient Medical Expenses

目的分析日间手术中心运行对住院医疗费用的影响,探讨日间手术的价值。方法 2017年1—12月,医院日间手术中心开展的三类手术作为研究对象,日间手术组:腹股沟疝115例,乳腺良性肿瘤156例,下肢静脉曲张105例。对照组:腹股沟疝124例、乳腺良性肿瘤170例、下肢静脉曲张110例。回顾性分析方法,收集三种手术的资料,进行费用对比分析。以腹股沟疝手术费用作为因变量,收集患者的资料,进行因素分析。结果腹股沟疝日间手术的床位费、护理费、药费、平均总费用低于非日间对象,差异有统计学意义(P <0.05)。乳腺良性肿瘤与下肢静脉曲张床位费、护理费、诊断费用、药费、平均总费用低于非日间手术,差异有统计学意义(P <0.05)。腹股沟疝、乳腺良性肿瘤、下肢静脉曲张的非日间手术床位费、诊断费与总费用存在相关性(P <0.05),腹股沟疝、乳腺良性肿瘤、下肢静脉曲张日间手术与非日间手术的护理费、药费与总费用存在相关性(P <0.05),日间手术的r相关系数低于非日间手术。结论日间手术确实可以减少住院治疗费用,但对不同类型的费用的影响不尽相同,在减少费用方面起到独立作用。

Objective To analyze the impact of the operation of day surgery centers on hospitalization costs and analyze the value of day surgery. Methods From January to December 2017, three types of surgery performed at the day surgery center of the hospital were used as the study subjects. The day surgery group included 115 inguinal hernias, 156 benign breast tumors, and 105 varicose veins of the lower extremities. The control group included 124 inguinal hernias, 170 benign breast tumors, and 110 varicose veins of the lower extremities. Retrospective analysis was performed to collect data from three operations and the cost was compared. The inguinal hernia surgery cost was used as a dependent variable to collect patient data and perform factor analysis. Results The bed fees, nursing fees, drug costs, and average total costs of inguinal hernia day surgery were lower than those of non-day subjects. The diference was statistically signifcant （P 〈 0.05）. The benign tumors and varicose veins in the lower limbs, nursing fees, diagnostic costs, drug costs, and average total cost were lower than non-day surgery, and the diference was statistically signifcant （P 〈 0.05）. There was a correlation between non-invasive surgery bed fees, diagnosis fees, and total expenses for inguinal hernia, benign breast tumors, and varicose veins of the lower extremities （P 〈 0.05）. Inguinal hernia, benign breast tumors, varicose veins of the lower extremities, between day surgery and non-day surgery There was a correlation between nursing costs, drug costs, and total costs （P 〈 0.05）. The r-correlation coefcient of daysurgery was lower than non-day surgery. Conclusion Day-time surgeries can indeed reduce hospitalization costs, but they have diferent efects on diferent types of costs and play an independent role in reducing costs.
