摘要
目的了解医患双方对柳州市医患关系现况的评价,为改善现有的医患关系提供建议。方法采用分层随机抽样方法,在柳州市不同等级医疗机构中选取15家医疗机构的1 037名医务工作人员和929名患者或家属进行问卷调查。结果患者或家属中认为医患关系的本质是商业与消费关系的比例远高于医务人员(24.1% vs 9.0%,P<0.000 1);34.8%的医务人员认为目前柳州医患关系差或很差,高于患者(7.2%,P<0.000 1)。医方认为导致医患关系紧张的第一原因是媒体负面报道,其次是患者期望值过高和医疗体制不合理;而患方认为前三位依次是医疗费用高、医患缺乏有效沟通以及医疗效果欠佳。医患双方均认为强化政府职责和增加对医院投入是改善或解决紧张医患关系的首要措施。结论医患双方对目前柳州市医患关系现况、原因及解决措施上持有各自的看法,存在差异,这可能直接影响医患关系的健康发展,因此,加强医患沟通、增进相互理解有利于缩小医患双方对医患关系的认知差异,有利于医患关系的改善。
Objective To compare the cognitive differences of the current doctor-patient relationship between hospital staffs and patients in Liuzhou,and promote a harmonious doctor-patient relationship.Methods A stratified random sampling method was used and finally 1 037 hospital staffs and 929 patients(or his/her family members)from 15 hospitals of Liuzhou were investigated using a self-designed questionnaire.Results 24.1% of patients considered the relationship between doctors and patients as a commercial relationship,which was much higher than the ratio among hospital staffs(9.0%)(P〈0.000 1).34.8% of hospital staffs thought the relationship between doctor and patient was bad,or even worse,higher than the patients' 7.2%(P〈0.000 1).Hospital staffs regarded the media with negative press,patients with high expectations and the unreasonable health care system as the top third reasons of difficult doctor-patient relationship.However,high cost of medical care,lack of effective communication between doctors and patients and poor treatment outcome were the main reasons chose by patients.Both of doctors and patients thought strengthen the government responsibilities and increase financial supports to hospital were the most two effectively measures to improve doctor-patient relationship.Conclusion There are quite different cognitive levels about doctor-patient relationship between doctor and patient,which may influences the construction of a harmonious doctor-patient relationship.Thus enhancing the effective communication between doctors and patients and mutual understanding will be very helpful for promoting a harmonious doctor-patient relationship.
