摘要
目的了解医患关系现状及其影响因素。方法分别采用量表PDRQ-15及DDPRQ-10问卷调查对患者1 769名及医务人员1 112名进行调查,采用描述性分析、独立样本t检验、非参数检验、Logistic回归分析等方法分析医患双方对医患关系评价差异及影响因素。结果患者评分较高,其中医患关系评价好1 097人(62%)、评价一般631人(35.7%)、评价差41人(2.3%);总分在年龄、户籍、是否参保、文化程度、经济水平、就诊类型上差异有统计学意义(P<0.05)。医务人员评分较低,其中医患关系评价好448人(40.3%)、评价一般598人(53.8%)、评价差66人(5.9%);总分在经济水平上差异有统计学意义(P<0.05)。Logistic回归分析显示,年龄越大、城镇户籍、参加医保、初中及以下教育程度、经济水平较高、住院的患者和认为经济水平处在中上水平的医务人员对医患关系总体评价较高。结论医患双方对医患关系的总体评价较好,但医方的自我感觉比患方评价差。建议理性看待目前的医患关系,及时疏导医务人员的负面情绪,通过健全医保体系、控制医疗费用、提升医患沟通及提高医务人员的合法收入等措施改善医患关系。
Objective To study the status and influencing factors of the doctor-patient relationship between medical personnel and patient.Methods Totally 1 769 patients and 1 112 medical personnel were investigated with the Patient-Doctor Relationship Questionnaire(PDRQ-15DDPRQ-10)respectively.The survey data was analyzed by descriptive analysis,independent sample t-test,non-parametric test and logistic regression analysis.Results Patients' scores were high.Of patients,1 097(62%)considered the doctor-patients relationship were good,631(35.7%)were general,41(2.3%)were poor.There were significant differences in age,household registration medicare,education,economic level and clinic type in the total score(P〈0.05).Medical personnel's score were low.And the good,general and poor attitude of doctor-patients were 448(40.3%),598(53.8%),and 66(5.9%),respectively.There were significant differences in economic level in the total score(P〈0.05).Logistic regression analysis showed the patients with older age,town census register,medicare,junior high school and below,higher income,hospitalization and the medical personnel with higher income had better evaluation of doctor-patient relationship.Conclusion It suggested to evaluate the current doctor-patient relationship rationally and ease the negative emotions of medical personnel in time.Establishing medical insurance system,reducing medical costs,improving doctor-patient communication,increasing the legitimate income of doctors are the efficient ways to improve the doctor-patient relationship.
