D-二聚体与凝血指标在不同孕期中的变化

Expressions and changes of D-Dimer and coagulation index in different pregnancy periods

目的:研究D-二聚体与凝血指标在不同孕期中的变化。方法:选取正常妊娠期妇女289例为妊娠组,选取同期正常非孕期妇女150例为对照组。按照孕期早、中、晚3个时期将妊娠组对象分成3个小组。对比两组的D-二聚体及凝血纤维蛋白原(fibrinogen,Fg)、活化部分凝血酶原时间(activated partial thromboplastin time,APTT)、凝血酶时间(thrombin time,TT)、凝血酶原时间(prothromin time,p T)四项指标。结果:妊娠组早中晚3个时期孕妇的年龄、孕次、产次差异无统计学意义(P> 0. 05);妊娠组的D-二聚体、Fg呈早、中、晚期呈递增趋势,p T呈递减趋势,APTT与TT晚期低于中、早期;各妊娠小组D-二聚体、Fb均高于对照组(P <0. 05),APTT、TT、p T均低于对照组(P <0. 05)。结论:不同孕期妇女的D-二聚体与凝血四项的指标水平不同,动态监测孕妇的D-二聚体与凝血有助于预防产中和产后的异常出血及并发症。

Objective： To study the expressions and changes of D-Dimer and coagulation index in different pregnancy periods. Methods： 289 normal pregnant women were selected as the pregnancy group,while 150 cases of normal non-pregnant women who were examined during the same period were selected as the control group. According to the three periods of early,middle and late pregnancy,the pregnant women were divided into three groups. The D-Dimer and four indexes of coagulation fibrinogen（ Fg）,activated partial thromboplastin time（ APTT）,thrombin time（ TT）,prothromin time（ p T） of the two groups were compared. Results：There was no statistical difference in the age,gravidity,parity of pregnant women in the three periods of early,middle and late pregnancy. The D-Dimer and Fg index increased progressively in the three periods of early,middle and late pregnancy. PT showed a declining trend. The APTT and TT in the late period were significantly lower than those in the middle and early periods. The D-Dimer and Fg index in pregnancy group were higher than those in the control group,and the three indexes of APTT,TT and p T were lower than those of the control group. Conclusion： The results of D-Dimer and four indexes of coagulation in different pregnant women are different. Dynamic monitoring of D-two dimer and coagulation in pregnant women can help to prevent abnormal bleeding and complications during childbirth and postpartum.
