超声乳腺影像报告和数据系统对小乳腺癌和乳腺增生的诊断价值

Diagnosis Value of Ultrasonic Breast Imaging Reporting and Data System in Diagnosis of Small Breast Cancer and the Hyperplasia of Mammary Gland

目的:探讨超声乳腺影像报告和数据系统(BI-RADS)对诊断小乳腺癌(SBC)与乳腺增生(HMG)的诊断价值。方法:回顾性选取2013年12月—2016年12月确诊的SBC与HMG患者各50例,均给予超声和钼靶检查,并以病理组织学检查为对照。结果:病理组织学检查显示,32例SBC伴腋窝淋巴结转移,SBC乳腺内钙化率(46.0%vs.2.0%)、形态不规则率(84.0%vs.48.0%)、边界蟹足样率(80.0%vs.18.0%)明显高于HMG。BI-RADS在诊断SBC与HMG上述表现符合率分别为100.0%和100.0%、92.9%和83.3%、95.0%和88.9%;钼靶分别为56.5%和0、71.4%和33.3%、60.0%和22.2%,差异有统计学意义(P<0.05);BI-RADS评估SBC腋窝淋巴结转移的符合率高于钼靶(93.7%vs.50.0%),差异有统计学意义(P<0.05)。结论:与钼靶比较,BI-RADS可提高鉴别SBC与HMG的诊断价值。

Objective： To explore diagnosis value of small breast cancer （SBC） and mammary hyperplasia （HMG） by ultrasonic breast imaging report and data system （BI-RADS）. Methods： From December 2013 to December 2016, 50 cases of SBC and HMG diagnosed in our hospital were selected. All patients were examined by ultrasonography and mammography, and the histopathological examination was used as a control. Results： Histopatho logical examination showed 32 cases of SBC with axillary lymph node metastasis. The calcification rate （46.0%vs.2.0%）, irregular rate （84% vs.48.0%） and border crab foot like rate （80% vs.18.0%） in SBC were significantly higher than that in HMG. The coincidence rate of BI-RADS in diagnosing SBC and HMG was 100.0% and 100.0%, 92.9% and 83.3%, 95.0% and 88.9%, respectively; molybdenum target was 56.5% and 0, 71.4% and 33.3%, 60.0% and 22.2%. The difference is statistically significant （P〈0.05）. The coincidence rate of BI-RADS in evaluating axiUary lymph node metastasis of SBC was higher than that of molybdenum target （93.7% vs.50.0%）, and the difference was statistically significant （P〈0.05）. Conclusion： Compared with molybdenum target, BI-RADS can improve the diagnostic value of SBC and HMG.
