带状疱疹继发瘢痕疙瘩

Keloids secondary to herpes zoster:a case report

报告1例带状疱疹继发瘢痕疙瘩。患者男,68岁。右侧胸背部红色隆起性斑块伴痛痒2年余。患者2年前无明显诱因下出现右侧前胸及后背异样感,伴发热、乏力,后逐渐形成局部水疱,误诊为烫伤,皮损治疗不当继发感染,并进一步出现瘢痕增生至我科就诊。皮肤科检查:右侧胸前及背部暗红色隆起性斑块,形状不规则,表面凹凸不平,向周围正常皮肤蟹足样延伸。体格检查及实验室检查未见异常。诊断:带状疱疹继发瘢痕疙瘩。给予复方倍他米松注射液每个月1次皮内注射,共6个月,联合醋酸地塞米松搽剂和索咪新尿素乳膏外用,隆起性斑块逐渐扁平萎缩。

A case of keloids secondary to herpes zoster is reported. A 68-year-old man complained of skin lesions on the right chest and back with pain and pruritus for more than 2 years. Two years ago, the patient started to have strange feeling on the right chest and back accompanied by fever and fatigue. Local blisters were gradually formed. He was misdiagnosed as scald. Because of inappropriate treatments, secondary infection occurred, following by formation of scar hyperplasia. Skin exam- ination showed raised irregular dark red plaques on the right chest and back, with crab-claw infiltration extending to normal tissues. Otherwise, both physical examination and laboratory tests were normal. Diagnosis of herpes zoster complicated with subsequent keloids was made. The patient was treated with intradermal betamethasone injection once monthly for six times and topical steroids.
