摘要
目的 探讨冠心病患者中血小板分布宽度(platelet distribution width,PDW)与冠状动脉狭窄程度的关系.方法 将经冠状动脉造影被诊断为冠心病的患者909例按照血小板分布宽度是否升高分为研究组360例(PDW≥16.3%),对照组549例(PDW< 16.3%).对冠状动脉造影结果进行SYNTAX Ⅰ积分以评估其狭窄程度,按照积分结果分为轻、中、重度三种狭窄等级.所有患者均进行年龄、性别、吸烟史、糖尿病史、血小板分布宽度(PDW)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、随机血糖(RBG)、同型半胱氨酸(Hcy)等基线资料对比之后,分析不同冠状动脉狭窄病变程度与PDW之间的关系.结果 研究组与对照组冠状动脉SYNTAX评分不同[(22.6±12.2)分比(19.7±10.9)分,t=3.70,P=0.000],轻、中、重度狭窄三组的PDW水平不同(Z=3.80,P=0.000).相关性分析显示,血小板分布宽度的水平与SYNTAX积分呈正相关(r=0.41),冠状动脉的狭窄程度随着PDW的升高而增加.多因素回归显示,年龄、PDW、TC是影响冠状动脉狭窄程度的主要因素.结论 血小板分布宽度与冠状动脉狭窄程度有一定程度的相关性,PDW可作为冠状动脉狭窄程度的一个快捷经济的预测因素.
Objective:To discuss the relationship between platelet distribution width(PDW) and the degree of coronary artery stenosis. Methods:909 patients who diagnosed with coronary artery disease(CAD) by percutaneous coronary intervention were divided into experimental group(n=360) with PWD level≥16.3% and control group(n=549) with their PDW level 〈16.3%. Patients were divided into three groups according to their SYNTAXⅠ Score.Their basic level of Age, Gender, Smoking history, diabetes,Hypertension,TC,TG,LDL,RBG,HCY were measured and compared. Assessment the relationship between platelet distribution width level and the degree of coronary artery stenosis. Results:The SYNTAX Score was higher in experimental group than in control group (22.6±12.16 vs19.74±10.9,t=3.7,p=0.000). The level of PDW is difference among the three groups (Z=3.8 P=0.000). Correlation analysis showed that PDW level was positively related with SYNTAX Score(r=0.41).The degree of coronary artery stenosis may aggravated with the increasing of PDW. Multiple regression analyses showed that age, PDW and TC were the main factors which can affect the degree of coronary stenosis. Conclusion:PDW has the extent relationship with the degree of coronary artery stenosis. PDW may serve as a simple and widely available predictor of extent of coronary artery stenosis.
