急性心肌梗死患者氧化低密度脂蛋白抗体测定价值评价

Evaluation of antibody to oxidized low-density lipoprotein antibody in patients with acute myocardial infarction

目的 探究在临床诊治急性心肌梗死患者的过程中,测定氧化低密度脂蛋白（ox-LDL）抗体的临床价值,为临床急性心肌梗死的诊治奠定基础.方法 选取80例冠心病患者作为观察组,21例健康人群作为对照组,检测患者ox-LDL与一氧化氮（NO）.结果 观察组血浆ox-LDL （687± 169）μg/L和循环内皮细胞计数（CEC）（9.0±1.7）xl06个/L明显高于对照组的正常水平,观察组NO水平（52.3±16.6）μmol/L则低于对照组（77.4±21.1）μmol/L.ox-LDL表达水平随着冠状动脉病变支数的增加而下降,但丙二醛（MDA）的水平则随之增加而增加,组织间差异显著（P＜0.05）.相关性分析显示,ox-LDL与NO对比（P=0.013、r=-0.782）,ox-LDL与CEC对比（P=0.019、r=0.793）,ox-LDL与MDA对比（P=0.002、r=-0.319）,CEC与NO对比（P=0.021、r=-0.952）.结论 在临床诊治急性心肌梗死患者的过程中,氧化低密度脂蛋白与急性心肌梗死患者的病情严重度密切相关,且其与血管内皮损伤程度密切相关,具有抗动脉粥样硬化的作用,因此,临床治疗急性心肌梗死过程中应重视对内皮细胞功能的保护与抗炎处理.

Objective To explore the clinical diagnosis and treatment in patients with acute myocardial in- farction and the clinical value of antibodies to oxidized low density lipoprotein, lay the foundation for clinical di- agnosis and treatment of acute myocardial infarction. Methods 80 cases of coronary heart disease patients as the observation group, 21 cases of normal people as control group, were detected with oxidized low density lipopro- tein and nitric oxide results： in the observation group. The plasma oxidized low density lipoprotein（687±169） μg/L and circulating endothelial cells （9.0±1.7） × 106/L was significantly higher than the normal level of the control group, the observation group the levels of nitric oxide （52.3 ± 16.6 ）μmol/L was lower than that of control group the expression level of ox-LDL decreased with the increase in the number of coronary arteries, but the MDA level was subsequently increased, the organization had significant difference （P〈0.05）. Correlation analysis showed that the comparison of ox-LDL and NO （P=0.013, r=-0.782）, ox-LDL and CEC （P=0.019, r=0.793） than in contrast, ox-LDL and MDA （P=0.002, r=-0.319）, CEC and NO compared （P=0.021, r=-0.952）. Conclusion In patients with acute myocardial infarction the level of oxidized low density lipoprotein was related with the se- verity of acute myocardial infarction and the vascular endothelial injury, therefore, for the treatment of acute myo- cardial infarction the protection and anti-inflammatory treatment on endothelial cell function should be pay atten- tion.
