刘尚义治疗肝胃不和证用药规律数据挖掘分析

Analysis of LIU Shangyi’s Clinical experience in Treating Liver Stomach Disharmony Syndrome Based on Data Mining

目的：应用古今医案云平台V1.3软件,分析探讨＂国医大师＂刘尚义教授治疗＂肝胃不和证＂临证用药特点。方法：筛选出2016年8月25日—2017年7月1日期间因＂肝胃不和证＂就诊于国医大师刘教授国医堂门诊的患者;将药方及相关信息录入软件,并采用关联规则、频次分析等方法,分析药物之间的关联规则及治疗原则等,从而分析探讨刘教授治疗＂肝胃不和证＂用药特点。结果：研究共录入医案421份,其中男135例,女286例;涉及中医疾病54种（胃痛、胁痛、腹痛、胃热、胃癌等）;涉及西医疾病28种（慢性胃炎、胃溃疡、反流性食管炎、胃癌、肠息肉等）;共涉及症状158个（胃痛、口干苦、反酸、纳差、胁痛等）;涉及方剂共23首（半夏泻心汤、小半夏汤、左金丸、三黄汤、黄连乌梅汤等）;涉及中药198味,总共使用频次6153次（黄连、吴茱萸、半夏、黄芩、郁金、佛手、徐长卿、木香等）;中药配伍模块分析,结果提示共获得15条中药组合关联规则;进行药对分析（前5位依次为：吴茱萸、黄连;黄连、半夏;黄连、黄芩;黄芩、吴茱萸;黄芩、半夏）。结论：刘教授治疗＂肝胃不和证＂多用疏肝行气,清热和胃之品。

Objective： To use the ancient and modern medical records cloud platform V1. 3 software to analyze and discuss Professor LIU Shangyi＇s clinical characteristics of liver-stomach disharmony syndrome. Methods：（1） We screened the patients who had liver-stomach disharmony syndrome from August 25,2016 to July 1,2017.（2）Enter the prescription and related information into the software. Association rules and frequency analysis were adopted. Association rules and treatment principles were analyzed so as to analyze and discuss the medication characteristics of Professor LIU＇s treatment of liver-stomach disharmony syndrome.Results： There was a total of 421 cases involved,135 males and 286 females,involving 54 kinds of Chinese medicine diseases（stomach pain,chest pain,abdominal pain,stomach cancer,etc.）,28 kinds of Western medicine diseases（chronic gastritis,gastric ulcer,reflux esophagitis,stomach cancer,intestinal polyps,etc.）. A total of 158 symptoms were involved（stomach pain,dry mouth pain,acid regurgitation,anorexia,hypochondriac pain etc.）. There were 23 prescriptions involved（Banxia Xiexin Decoction,Xiaobanxia Decoction,Coptis Decoction,Zuojin Pill,Huanglina Wumei Decoction etc.）,involving 198 kinds Chinese medicine. The total frequency was 6153 times（Wu Zhuyu,Coptis,pinellia,Radix Scutellariae,radix curcumae,bergamot,xuchangqing,wood etc.）. Fifteen association rules of Chinese herbal medicine were obtained. Drug pair analysis： The top 5 were Wu Zhuyu,Rhizoma Coptidis; Rhizoma Coptidis,Pinellia ternate; Rhizoma Coptidis,Scutellaria baicalensis Georgi; Scutellaria baicalensis Georgi,Wu Zhuyu; Scutellaria baicalensis and Pinellia ternate. Conclusion： Professor LIU treated liver-stomach disharmony syndrome with soothing the liver and promoting Qi and clearing away stomach heat method.
