复方苦参注射液对同步放化疗肺癌患者放射性肺损伤及生活质量的影响

Effect of Compound Radix Sophorae Flavescentis Injection on Radiation Lung Injury and Quality of Life in Lung Cancer Patients Undergoing Concurrent Chemoradiotherapy

目的：观察复方苦参注射液对紫杉醇同步适形放疗（intensity modulated radiation therapy,IMRT）治疗局部晚期非小细胞肺癌患者（non-small lung cancer,Nsclc）的放射性肺损伤和生活质量的影响。方法：86例非小细胞肺癌患者均为局部晚期,随机分为对照组和观察组,44例观察组患者同步放化疗时联合应用复方苦参注射液,42例对照组患者同步放化疗时不应用复方苦参注射液及其他可能对放射性肺损伤及生活质量有影响的药物。结果：两组患者均完成了同步放化疗治疗计划,按照RTOG放射性肺损伤评价标准,对照组I-Ⅱ级急性和慢性放射性肺损伤的发生率均明显高于观察组（P〈0.05）;同步放化疗治疗过程中及治疗后,两组患者生活质量的改变均有明显差异（P〈0.05）。结论：同步放化疗时联合应用复方苦参注射液治疗局部晚期非小细胞肺癌,可以明显减少放射性肺损伤的放射率并降低放射性肺损伤的严重程度,明显提高患者放疗中及放疗后的生活质量。

Objective： To observe the effect of intensity modulated radiation therapy（IMRT） on radiation lung injury and quality of life in patients with locally advanced non-small lung cancer（Nsclc）. Methods： Eighty-six patients with non small cell lung cancer were locally advanced. They were randomly divided into control group and observation group. Fourty-four cases of observation group were treated with compound Sophora flavescens injection during concurrent chemoradiotherapy. Fourty-two cases of control group should not be treated with compound matrine injection or other drugs which may affect radiation-induced lung injury and quality of life. Results： The two groups had completed the synchronous radiotherapy and chemotherapy treatment plan. According to the RTOG radionuclide injury evaluation standard,the incidence of I-II acute and chronic radioactive lung injury in the control group was significantly higher than that of the observation group（P〈0. 05）. The changes of the quality of life of the two groups were significantly different in the course of chemotherapy and treatment（P〈0. 05）. Conclusion： Combined use of compound injection of Sophora Flavescentis Injection in the treatment of locally advanced non-small cell lung cancer can significantly reduce the radiation rate of radioactive lung injury and reduce the severity of radioactive lung injury,and improve the quality of life in patients with radiotherapy and radiotherapy.
