摘要
目的:观察电针对去卵巢大鼠膝软骨下骨及关节软骨的影响。方法:将72只清洁级Sprague-Dawley大鼠随机分为假手术组、去卵巢组和电针组,每组24只。去卵巢组及电针组大鼠施行双侧卵巢切除术。去卵巢术后,电针组给予电针治疗,每天1次,每周5 d。干预12周,分别在第4、8、12周各组随机处死8只大鼠,检测尿I型胶原C端肽(Urinary C-terminal cross-linking telopeptide of type I collagen,CTX-I)、尿II型胶原C端肽(Urinary C-terminal cross-linking telopeptide of type II collagen,CTX-II),血清雌二醇(serum 17β-oestradiol,E2),对软骨下骨行显微CT及骨组织定量分析,对关节软骨行Mankin评分。结果:(1)生化检测:在术后4、8、12周,去卵巢组的尿CTX-I均比假手术组高(P〈0.01,P〈0.01,P〈0.01),电针组的尿CTX-I均比去卵巢组降低(P〈0.01,P〈0.05,P〈0.01);去卵巢组的尿CTX-II比假手术组高(P〉0.05,P〈0.01,P〈0.01),电针组的尿CTX-II比去卵巢组降低(P〉0.05、P〈0.05、P〈0.05),除术后4周,差异均具有统计学意义。去卵巢组的血清E2均比假手术组低(P〈0.01,P〈0.01,P〈0.01),电针组的血清E2均比去卵巢组高(P〈0.01、P〈0.01、P〈0.01),差异均有统计学意义。(2)显微CT检测:在术后4、8、12周,去卵巢组的骨体积分数(BV/TV)和骨小梁厚度(Tb.Th)均比假手术组低(P〈0.01,P〈0.01,P〈0.01)、(P〈0.01,P〈0.05,P〈0.01),电针组的BV/TV和Tb.Th均比去卵巢组高(P〈0.05,P〈0.01,P〈0.01)、(P〈0.01,P〈0.05,P〈0.01);去卵巢组的骨小梁数量(Tb.N)均比假手术组减少,但只在术后8、12周有统计学差异(P〈0.05,P〈0.01);电针组的Tb.N比去卵巢组增多,但只在术后12周有统计学差异(P〈0.05);去卵巢组的骨小梁分离度(Tb.Sp)均比假手术组高(P〈0.01,P〈0.05,P〈0.01),以上差异均有统计学意义;在术后4、12周,电针组的Tb.Sp比去卵巢组低,差异有统计学意义(P〈0.05,P〈0.01)。(3)软骨组织形态检测:在术后4、8周,去卵巢组的Mankin评分比假手术组高(P〉0.05),电针组的Mankin评分比去卵巢组低(P〉0.05),差异均无统计学意义;在术后12周,去卵巢组的Mankin评分比假手术组高(P〈0.01),电针组的Mankin评分比去卵巢组低(P〈0.01),差异有统计学意义。结论:电针可抑制去卵巢大鼠的软骨下骨骨质疏松及关节软骨退化。
Objective: To observe the effects of electroacupuncture(EA) on subchondral bone and cartilage degeneration in an experimental animal model of osteoarthritis induced by ovariectomy. Methods: Seventy-two 3-month-old female Sprague-Dawley rats were randomly divided into the following 3 groups(n = 24); sham operation without treatment(Sham-operated group),ovariectomy without treatment(OVX group),and ovariectomy with EA reatment(EA group). Rats in EA group received EA treatment from the day of OVX,8 rats in each group were randomly killed at the 4 th,8 th,12 th week after operation. To value urinary CTX-I,Urinary CTX-II and E2 by Biochemical test,to value subchondral bone by Micro-computed tomography and histology analysis,to value cartilage by Mankin score. Results:(1) Biochemical test: At the 4 th,8 th,12 th week after operation,Urinary CTX-I of OVX group was higher than those in Sham-operated group(P〉0. 01,P〈0. 01,P〈0. 01). Urinary CTX-I of EA group was lower than those in OVX group(P〉0. 01,P〈0. 05,P〈0. 01). Urinary CTX-II of OVX group was higher than those in Sham-operated group(P〈0. 05,P〈0. 01,P〈0. 01). Urinary CTX-II of EA group was lower than those in OVX group(P〈0. 05,P〈0. 05,P〈0. 05). E2 of OVX group was lower than those in Sham-operated group(P〈0. 01,P〈0. 01,P〈0. 01). E2 of EA group were higher than those in OVX group(P〈0. 01,P〈0. 01,P〈0. 01).(2) Micro-computed tomography and histology analysis: At the 4 th,8 th,12 th week after operation,BV/TV and Tb. Th of OVX group were lower than those in Sham-operated group(P〈0. 01,P〈0. 01,P〈0. 01)(P〈0. 01,P〈0. 05,P〈0. 01); BV/TV and Tb. Th of EA group were higher than those in OVX group(P〈0. 05,P〈0. 01,P〈0. 01)(P〈0. 01,P〈0. 05,P〈0. 01); Tb. N of OVX group was lower than those in Sham-operated group,but only at the 8 th,12 th week after operation was statistically significant(P〈0. 05,P〈0. 01). Tb. N of EA group was higher than those in OVX group,but only at the 12 th week after operation was statistically significant(P〈0. 05); Tb. Sp of OVX group was higher than those in Sham-operated group(P〈0. 01,P〈0. 05,P〈0. 01). At the 4 th,12 th week after operation,Tb. Sp of EA group were lower than those in OVX group(P〈0. 05,P〈0. 01).(3) Cartilage morphology test: At the 4 th,8 th week after operation,Mankin score of OVX group were higher than those in Sham-operated group(P〉0. 05),and that of EA group were lower than those in OVX group(P〉0. 05). At the 12 th week after operation,Mankin score of OVX group was higher than those in Sham-operated group(P〈0. 01) and those of EA group was lower than those in OVX group(P〈0. 01). Conclusion: The results showed that EA could inhibit subchondral bone deterioration and ameliorates cartilage degeneration in ovariectomized rats.
