摘要
目的针对中国人群,探讨最适合测量巨大儿出生体重的方法。方法密切随访四川大学华西第二医院就治的孕晚期孕妇,对于顺产的600例巨大儿的临床资料进行回顾性分析,使用三种产科常见胎儿体重计算方式,估计胎儿体重,比较三种测量方法估算的胎儿体重与实际新生儿出生体重的差异性。结果使用"HadlockIII"公式计算的胎儿体重与实际出生后新生儿体重的误差最小。同时,使用"HadlockIII"公式计算的平均绝对误差、平均相对误差最小。结论对于临床上疑似巨大儿的病例,使用HadlockIII计算胎儿体重的方法比其他各种方法更接近出生体重,准确性更高。
Objective The purpose of our research is to compare the accuracy of different weight formulas for predicting macrosomia fetal birth weight at term infants in Chinese population.Methods The retrospective study was conducted at Obstetrics Department of West China Second University Hospital and included 600 gestation women delivered macrosomia.Full fetal biometry parameters were measured within 1 week of delivery by the monographers.We used 3 different formulas to evaluate the fetal weight and analyze the best calculate system.Results A total of600 pregnant women were included in our study.The accuracy of different formulas to different race and weight are completely different.Across the weight range from 4000 to 5000,Hadlock III had the best intraclass correlation coefficients and accuracy results.Conclusion In the clinic,the accurate fetal assessment can provide crucial and useful reference for the delivery methods,delivery time and eventually avoiding severe vaginal injury.
