自拟降糖脑通方治疗糖尿病性脑梗死的临床研究

Clinical research of diabetic cerebral infarction treated with the self-prepared jiangtang naotong formula

目的探讨自拟降糖脑通方对糖尿病性脑梗死患者炎性细胞因子及血管内皮生长因子(VEGF)的影响。方法选取2015年5月至2016年11月期间收治的138例糖尿病性脑梗死患者为研究对象,根据随机数表法分为研究组与对照组,均69例。对照组给予患者常规西药抗血小板凝集、降血脂、降血糖等治疗,研究组患者在对照基础上应用自拟降糖脑通方治疗。比较两组治疗前后炎性细胞因子[白细胞介素-6(IL-6)、可溶性细胞间黏附分子-1(s ICAM-1)、C反应蛋白(CRP)]水平及VEGF、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]情况,并评价两组临床疗效。结果两组患者治疗后,研究组临床疗效较对照组明显更好,差异具有统计学意义(P<0.05)。两组患者治疗后VEGF水平均较治疗前上升,IL-6、s ICAM-1、CRP水平及NIHSS评分均较治疗前下降(P<0.05),且研究组情况均优于对照组,差异具有统计学意义(P<0.05)。结论对糖尿病性脑梗死患者采用自拟降糖脑通方治疗,可有效提高临床疗效,降低患者炎性细胞因子水平,提高VEGF水平,促进神经功能的恢复,优势显著,值得进一步推广应用。

Objective To explore the impacts of the self-prepared jiangtong naotong formula on the inflammatory cytokines and the vascular endothelial growth factor（VEGF） in the patients of diabetic cerebral infarction. Methods From March 2015 to November 2016,138 patients of diabetic cerebral infarction were collected as the subjects. According to the random number table,they were divided into a research group and a control group,69 cases in each one. In the control group,the routine western medication was used for antiplatelet aggregation,reducing blood lipid and antidiabetics. In the research group,on the base of the treatment as the control group,the self-prepared jiangtang naotong formula was used. The levels of inflammatory cytokines（IL-6,s ICAM-1 and CRP）,VEGF and neurological function（NIHSS score）were compared between the two groups before and after treatment. The clinical efficacy was assessed in the two groups. Results After treatment,the clinical effect in the research group was obviously better than the control group,indicating the significant difference（P〈0. 05）. VEGF level was increased after treatment as compared with that before treatment in the two groups,and the levels of IL-6,s ICM-1 and CRP,as well as NIHSS score were all reduced as compared with those before treatment（P〈0. 05）. The results in the research group were better than the control group,indicating the significant differences（P〈0. 05）. Conclusion The self-prepared jiangtong naotong formula effectively improves the clinical efficacy on diabetic cerebral infarction,reduces the levels of inflammatory cytokines,improves VEGF level and promotes the recovery of the neurological function. This therapy presents the significant advantages in treatment and deserves to be further promoted in application.
