摘要 目的探讨超声Graf法对婴儿发育性髋关节发育不良(DDH)的诊断价值。方法选择在医院出生或就诊且具备DDH高危因素的90例婴儿,采用Graf法测定婴儿髋关节形态、结构,并进行髋关节Graf分型,分析超声Graf法对婴儿DDH的筛查价值。结果 (1)90例婴儿共180个髋关节纳入研究,超声检出Ⅱc型及以上共42髋,异常检出率为23.33%;(2)DDH患儿髋关节超声测定α角明显小于正常髋关节,β角明显大于正常髋关节(P <0.05)。结论超声Graf法无创,可重复性好,对软骨及软组织分辨率高,可显示婴儿髋臼发育不良情况及脱位程度,可作为DDH筛查的常用手段。 Objective To explore the diagnostic value of ultrasound Graf for infant developmental dysplasia of the hip (DDH). Methods A total of 90 infants who had been born or treated in our hospital and had had high risk factors of DDH were selected. Graf was taken to determine the shape and structure of infants' hip. Hip Graf typing was conducted to analyze the screening value of ultrasound Graf for infant DDH. Results (1) A total of 180 hips of 90 infants were included in the study. A total of 42 hips of Ⅱ c type or above were detected by ultrasound, with an abnormal detection rate of 23.33%; (2) The angle α determined by ultrasound in patients with DDH was significantly smaller than that of normal hip, while the angle β was significantly greater than that of normal hip (P 〈 0.05). Conclusion Ultrasound Graf is noninvasive, has good repeatability, has high resolution of cartilage and soft tissue, can show the acetabular dysplasia and degree of dislocation in infants, and can be used as a common means of DDH screening.
