养阴清热法治疗外邪内袭所致小儿寰枢关节半脱位的疗效研究

Therapeutic Effect of Nourishing Yin and Clearing Heat on Infantile Atlantoaxial Joint Subluxation Caused by Invaded the Exogenous Svil

目的观察养阴清热法治疗外邪内袭所致小儿寰枢关节半脱位的有效性。方法对由外邪内袭所致小儿寰枢关节半脱位的60例患儿随机分为对照组及治疗组,对照组采用持续枕颌带牵引法治疗,治疗组在对照组的基础上配合根据养阴清热法而拟的中药。从视觉模拟评分法(VAS)评定患儿颈部疼痛变化情况、寰枢椎旋转活动角度,影像学数据(VBLADS)及开口位寰齿前间距(ADI)等方面系统观察研究。结果治疗组与对照组治疗前对比VAS积分、寰枢椎旋转活动度数、影像学数据(VBLADS、ADI)无显著性差异(P> 0.05)。治疗后两组VAS积分、寰枢椎旋转活动度数、影像学数据(ADI)有统计学差异(P <0.05),影像学数据(VBLADS)无统计学差异(P> 0.05)。两组治疗前后VAS积分、寰枢椎旋转活动度数、影像学数据(VBLADS、ADI)有显著性差异(P <0.05)。结论养阴清热法可有效治疗外邪内袭型小儿寰枢关节半脱位,能改善患儿临床症状和体征、缩短患儿的治疗周期。

Objective To observe the effectiveness of Nourishing Yin andClearing Heat in treating Pediatric subatlantic joint subluxation caused byInvaded the exogenous evil. Methods Sixty cases of Pediatric AtlantoaxialJoint Subluxation caused by invaded the exogenous evil were randomlydivided into control group（30 cases） and treatment group（30 cases）. Thecontrol group was treated with continuous occipital-jaw traction and thetreatment group was given the therapy of nourishing yin and clearing heatand the proposed Chinese medicine on the basis of the treatment for thecontrol group. The VAS method was used to assess changes in neck pain,angle of atlantoaxial rotation, VBLADS and ADI were observed beforeand after treatment. Results There was no significant difference in the VASscore, the rotational activity degree of the atlantoaxial, and the imagingdata （VBLADS, ADI） between the treatment group and the control groupbefore treatment（P 〉 0.05）, After treatment, the differences in VAS scores,the rotational activity degree of the atlantoaxial and imaging data （ADI）between the two groups was statistically signifycant （P 〈 0.05）, and therewas no significant difference in imaging data （VBLADS） （P 〉 0.05）. Thedifferences in VAS score, the rotational activity degree of the atlantoaxial,and imaging data （VBLADS, ADI） before and after treatment in bothgroups was statistically significant （P 〈 0.05）. Conclusion The therapy ofnourishing yin and clearing heat can effectively treat pediatric atlantoaxialjoint subluxation caused by invaded the exogenous evil, can improve theclinical symptoms and signs of child, and shorten the treatment cycle ofchild.
