冠心病患者同型半胱氨酸与冠脉病变严重程度关系研究

摘要 目的探讨冠心病患者同型半胱氨酸(Hcy)与冠脉病变严重程度的相关性。方法以2015年1月—2017年12月该院50例冠脉CT或造影确诊的冠心病患者为实验组,另择取同期冠脉CT或造影确诊冠脉狭窄但未至冠心病标准的50例患者为对照1组,50名冠脉正常的健康志愿者为对照2组。抽取3组静脉血液样本测定血清Hcy水平,基于Gensini系统评分,Pearson分析其与冠脉病变的相关性。结果实验组Gensini评分(27.6±5.2)分、血Hcy(24.5±6.7)μmol/L高于对照1组,对照1组Gensini评分和血Hcy水平高于对照2组,差异均有统计学意义(P<0.05),3组血Hcy水平随Gensini系统评分增高呈上升趋势,Pearson线性分析两者呈正相关(r=0.813,P=0.016)。结论 Hcy与冠脉病变严重程度存在正相关性,在冠心病风险评估中具有较高的预测价值,定期监测其水平能够为本病临床诊治提供可靠依据,值得临床推广使用。 Objective To investigate the relationship between homocysteine (Hcy) and the severity of coronary artery disease in patients with coronary heart disease. Methods From January 2015 to December 2017, 50 patients with coronary heart disease diagnosed by coronary CT or angiography in the hospital were enrolled in the study group. The patients who underwent coronary CT or angiography to confirm coronary stenosis but did not reach the standard of coronary heart disease 2. The patients were in the control group 1 and 50 healthy volunteers with normal coronary artery were the control group. Three groups of venous blood samples were taken to determine serum Hcy levels. Based on the Gensini system score, Pearson analyzed the correlation with coronary lesions. Results The Gensini score (27.6±5.2)points and blood Hcy (24.5±6.7)μmol/L in the experimental group were higher than those in the control group. The Gensini score and blood Hcy level in the control group were higher than those in the control group. The difference was statistically significant (P〈0.05), the levels of Hcy in the three groups increased with the increase of the Gensini system score, and Pearson linear analysis showed a positive correlation (r=0.813, P=0.016). Conclusion There is a positive correlation between Hcy and the severity of coronary artery disease, and it has a high predictive value in the risk assessment of coronary heart disease. Regular monitoring of its level can provide a reliable basis for the clinical diagnosis and treatment of this disease, and it is worthy of clinical promotion.
