摘要 目的探讨影像对比在膝关节损伤诊断中的临床应用价值。方法选取2011年1月—2015年1月甘肃省高台县人民医院收治的78例膝关节损伤患者,对入选损伤患者通过进行X线、CT、再行核磁共振成像(MRI)检查,同时结合临床,采用回顾性分析的方法对其影像表现进行分析。结果 78例膝关节损伤患者中经常规X线检查发现半月板损伤20例,占25.64%;骨损伤10例,占12.82%;膝关节韧带损伤22例,占28.20%,未发现26例,占33.33%;后经CT检查发现膝关节损伤半月板损伤24例,占30.77%;骨损伤12例,占15.38%;膝关节韧带损伤26例,占33.33%,未发现16例,占20.51%;最后行MRI检查半月板损伤28例,占35.90%;骨损伤16例,占20.51%;膝关节韧带损伤32例,占41.23%,未发现2例,占2.56%。X线及CT在检出率的比较上,差异无统计学意义(χ~2=3.260,P>0.05);MRI在检出率的比较上,分别优于X线及CT(χ~2=25.141、12.389,P<0.05)。结论 MRI对临床各种膝关节损伤的诊断准确性较高,属于一种无创伤性检查性方式,它既能反应患者膝关节的病理变化和变化程度,可对临床方案的制定及预后的判断提供及时、准确、全面的影像学参考。 Objective To investigate the clinical value of image contrast in the diagnosis of knee joint injury. Methods A total of 78 patients with knee joint injuries admitted to Gaotai County People's Hospital of Gansu Province from January 2011 to January 2015 were enrolled. X-ray, CT, and MRI were performed on patients with injury. The image performance was analyzed by retrospective analysis. Results In 78 patients with knee joint injury, 20 cases of meniscus injury were found by routine X-ray examination, accounting for 25.64%; 10 cases of bone injury accounted for 12.82%; 22 cases of knee joint ligament injury, accounting for 28.20%, 26 cases were not found. 33.33%; after CT examination, 24 cases of knee joint injury were found to have meniscus injury, accounting for 30.77%; 12 cases of bone injury, accounting for 15.38%; 26 cases of knee ligament injury, accounting for 33.33%, 16 cases were not found, accounting for 20.51%; At the end of the line, MRI was used to detect 28 cases of meniscus injury, accounting for 35.90%; 16 cases of bone injury, accounting for 20.51%; 32 cases of knee ligament injury, accounting for 41.23%, 2 cases were not found, accounting for 2.56%. There was no statistically significant difference in the detection rate between X-ray and CT (χ2=3.260, P〉0.05). The comparison of MRI was better than X-ray and CT (χ2=25.141, 12.389, P〈0.05). Conclusion MRI has high diagnostic accuracy for various knee injuries. It is a non-invasive examination method. It can not only reflect the pathological changes and changes of knee joints, but also determine the clinical plan and prognosis and provide timely, accurate and comprehensive imaging reference.
