PFNA内固定治疗老年不稳定股骨粗隆间骨折的疗效和安全性分析

Analysis of Curative Effect and Safety of PFNA Internal Fixation in Treatment of Unstable Intertrochanteric Fracture of Femur of Senile Patients

目的研究防旋股骨近端髓内钉内固定治疗年不稳定股骨粗隆间骨折的临床效果,并分析其安全性。方法选择2015年2月—2016年8月期间该院收治的34例老年不稳定股骨粗隆间骨折患者作为研究组病例,应用防旋股骨近端髓内钉内固定治疗,选择2010年2月—2011年8月期间该院收治的34例老年不稳定股骨粗隆间骨折患者作为对照组病例,应用常规内固定手术治疗,对比分析两组患者的手术临床效果与不良反应发生情况。结果研究组显效19,治疗总有效率为91.18%,对照组显效13例,治疗总有效率为73.53%,研究组中1例患者发生深静脉血栓,1例继发性骨折,手术后并发症发生率为5.88%,与对照组(26.47%)比较,组间差异有统计学意义(χ~2=5.835 0、5.314 2,P=0.015 7、0.021 1)。结论老年不稳定股骨粗隆间骨折患者应用防旋股骨近端髓内钉内固定治疗的效果显著,值得临床应用探索。

Objective To research the curative effect and safety of PFNA internal fixation in treatment of unstable intertrochanteric fracture of femur of senile patients. Methods 34 cases of senile patients with unstable intertrochanteric fracture of femur admitted and treated in our hospital from February 2015 to August 2016 were selected as the research group, treated with proximal femoral nail anti-rotation internal fixation, while 34 cases of senile patients with unstable intertrochanteric fracture of femur admitted and treated in our hospital from February 2010 to August 2011 were selected as the control group, treated with routine internal fixation surgery, and the clinical effect and occurrence of adverse reactions were compared between the two groups. Results In the research group, 19 cases were markedly effective, and the total treatment effective rate was 91.18% in the control group, 13 cases were markedly effective, and the total treatment effective rate was 73.53%, and in the research group, 1 case was with deep vein thrombosis, 1 case was with secondary fracture, and the incidence rate of complications after surgery in the research group and in the control group was respectively 5.88% and 26.47%, and the difference was obvious, with statistical significance（χ~2=5.835 0, 5.314 2, P=0.015 7, 0.021 1）. Conclusion The application effect of PFNA internal fixation in treatment of unstable intertrochanteric fracture of femur of senile patients is obvious, which is worth clinical application and exploration.
