摘要
目的在宫颈病变患者的筛查过程中,通过宫颈脱落细胞DNA定量分析的方法进行筛查,将取得的临床价值进行研究分析。方法随机抽取该院在2016年1月—2018年3月,这一期间内收治的500例病例,将所有患者作为该次研究的研究对象,分别分成两组,每组各有250例,一组叫做DNA组,一组叫做TCT组,DNA组患者通过宫颈脱落细胞DNA定量分析方法对宫颈病变患者进行筛查,TCT组患者通过液基薄层细胞学(TCT)方法对宫颈病变患者进行筛查,最后将两种方法进行筛查得出的结果进行比较,然后将结果展开分析。结果该次研究所取的全部500例病例,通过DNA定量分析方法,最后筛查出来宫颈病变为阳性患者有21例,DNA组中患者得到的阳性检出率、阳性预测值、阴性预测值、诊断准确率、灵敏度和特异性,分别为8.40%、95.16%、23.04%、72.41%,72.41%和74.79%;通过TCT法进行筛查TCT组中筛查出来宫颈病变为阳性患者的有9例,TCT组患者得到的阳性检出率、阳性预测值、诊断的准确率、阴性预测值、灵敏度和特异性,分别为3.60%、42.98%、56.25%、65.97%,56.25%、38.76%,两组之间的数据的差异有统计学意义(P<0.05)。两组的阳性检出率以及诊断准确率的差异有统计学意义(χ~2=5.264 9、15.246 1)。结论通过DNA定量分析的方法对宫颈病变进行筛查,得到的病理诊断的准确率、阳性预测值、敏感度、特异度均比通过TCT方法进行筛查得到的结果更高、更准确,表明DNA定量分析对宫颈病变进行筛查的有效性。
Objective To investigate the clinical value of DNA quantitative analysis of cervix in the screening of cervix lesions. Methods 500 cases of patients admitted and treated in our hospital from January 2016 to March 2018 were randomly divided into two groups with 250 cases in each, the DNA group screened the cervix lesions by the cervical exfoliative cells DNA quantitative analysis, while the TCT group screened the lesions by the TCT method, and finally,the results were compared and analyzed. Results Of 500 cases, 21 cases of positive cervix lesion patients were screened finally, and the positive test, positive predication value, negative predicative value, diagnosis accurate rate,sensitivity and specificity in the DNA group were respectively 8.40%,95.16%, 23.04%,72.41%,72.41%, 74.79%, and the TCT method showed that there were 9 cases of positive cervix lesion patients, and the positive test, positive predication value, negative predicative value, diagnosis accurate rate, sensitivity and specificity in the TCT group were respectively 3.60%, 42.98%, 56.25%, 65.97%, 56.25%, 38.76%, and the differences were obvious, with statistical significance, P0.05, and the positive test rate and diagnosis accuracy rate in the two groups,the differences were statistically significant(χ~2=5.264 9, 15.246 1). Conclusion The accurate rate, positive predictive value, sensitivity, and specificity of DNA quantitative analysis of cervix lesions are higher and more accurate than those of TCT method,which shows that the DNA quantitative analysis can effectively screen the cervix lesions.
