全胸腔镜肺叶肺段切除术围手术期护理体会

Perioperative nursing experience of total t horacoscopic lobectomy and segmentectomy

目的分析总结全胸腔镜肺叶肺段切除术在围术期的护理体会。方法回顾性分析2017年1～12月期间20例于本院行全胸腔镜肺叶肺段切除术治疗的患者作为本次研究对象,通过对患者行围术期护理,在术前加强健康宣教,指导患者咳嗽及深呼吸锻炼,做好呼吸道护理,加强患者的生命体征及呼吸道护理,密切关注患者的生命体征及引流管。结果患者在术后的平均引流时间为(3.1±1.3)d,术后平均住院时间为(7.1±2.5)d,均痊愈未出现严重围术期并发症。所有患者均痊愈出院。患者护理前后满意度也明显提升,相较护理前存在显著差异(P<0.05)。患者护理前生活质量评分为(68.51±12.29)分,护理后生活质量评分为(82.67±14.26)分,护理前后存在显著差异(P<0.05)。结论通过对行全胸腔镜肺叶肺段切除术治疗的患者,行围手术期护理干预模式,可以有效预防并发症的发生率,保持患者的胸腔闭式引流畅通,促进患者能够提高术后恢复速率,提高患者的生存质量,可以在临床中推广使用。

Objective To analyze and summarize the nursing experience of perioperative period of total thoraeoseopie lobectomy and segmentectomy. Methods A total of 20 patients who underwent total thoracoscopic lobectomy and seg- mentectomy in our hospital from January to December 2017 were retrospectively analyzed. Perioperative care for patients was performed. The health education before surgery was strengthened. The patients were guided to cough and do deep breathing exercise. The respiratory care was done well. The patients＂ vital signs and respiratory care was strength- ened, and the patients＂ vital signs and drainage tubes were paid close attention to. Results The postoperative average drainage time of the patients was（3.1±1.3） days, and the average postoperative hospital stay was（7.1±2.5） days. There were no serious perioperative complications. All patients were cured and discharged. The satisfaction of patients after nursing was also significantly higher, which was significantly different from that before nursing （P〈0.05）. The patient＇s pre-care quality of life score was（68.51±12.29）, and the post-nursing quality of life score was（82.67±14.26）. There was a significant difference in the quality of life before and after nursing（P〈0.05）. Conclusion Perioperative nursing inter- vention mode in patients treated with total thoracoscopic lobectomy and segmentectomy can effectively prevent the incidence of complications, maintain the patient＇s closed thoracic drainage smooth, and promote the recovery rate of patients after surgery, which improves the quality of life of patients, and can be promoted in clinical use.
