酸枣仁汤加减治疗对维持性血液透析伴抑郁症患者临床症状及睡眠质量的影响

Effects of Addition and Subtraction Suanzaoren Decoction Treatment on Clinical Symptoms and Sleep Quality for Patients with Maintenance Hemodialysis Complicated with Depression

目的:观察维持性血液透析(MHD)伴抑郁症患者采用酸枣仁汤加减治疗,对改善临床症状及睡眠质量作用。方法:研究选取2016年6月~2017年12月我院收治的MHD伴抑郁症患者68例,按随机数字表法分成研究组34例与对照组34例,对照组口服盐酸马普替林片与艾司唑仑片,研究组进行酸枣仁汤加减治疗,比较2个月后疗效。结果:治疗2月后,两组中医症候积分均显著低于治疗前(P<0. 05),且研究组改善程度优于对照组(P<0. 05);研究组中医症候疗效总有效率94. 12%,显著高于对照组的73. 53%(P<0. 05);两组HAMD评分均显著低于治疗前(P<0. 05),且研究组改善程度优于对照组(P<0. 05);两组PSQI评分均显著低于治疗前(P<0. 05),且研究组改善程度优于对照组(P<0. 05);两组血浆5-HT及MT水平均显著高于治疗前(P<0. 05),且研究组改善程度优于对照组(P<0. 05);治疗期间,研究组不良反应发生率2. 94%,显著低于对照组不良反应发生率26. 47%(P<0. 05)。结论:酸枣仁汤加减治疗用于MHD伴抑郁症患者,可以减轻抑郁程度,改善睡眠质量。

Objective： To investigate the effect of Addition and Subtraction Suanzaoren Decoction on the improvement of clinical symptoms and sleep quality for patients with maintenance hemodialysis（ MHD） complicated with depression. Methods：68 cases of MHD patients with depression admitted to our hospital from June 2016 to December 2017 were selected for the study and divided into study group（ n = 34） and controlled group（ n = 34） according to the random number table method. The controlled group was orally given Maprotiline Hydrochloride Tablets and Estazolam Tablets,and the study group was treated with Suanzaoren Decoction. The efficacy after 2 months was compared. Results： After 2 months of treatment,the scores of TCM symptoms in the groups were significantly lower than those before treatment（ P〈0. 05）,and the improvement in the study group was better than that in the controlled group（ P〈0. 05）. The total effective rate of TCM syndromes efficacy in the study group was significantly higher than that in the controlled group（ 94. 12%vs73. 53%）（ P〈0. 05）. The HAMD scores in the two groups were significantly lower than those before treatment（ P〈0. 05）,and the improvement in the study group was better than that in the controlled group（ P〈0. 05）. The PSQI scores in the two groups were significantly lower than those before treatment（ P〈0. 05）,and the improvement in the study group was better than that in the controlled group（ P〈0. 05）. The levels of plasma 5-HT and MT were significantly higher than those before treatment（ P〈0. 05）,and the improvement in the study group was better than that in the controlled group（ P〈0. 05）. During treatment,the incidence rate of adverse reactions in the study group was significantly lower than that in the controlled group（ 2. 94% vs26. 47%）（ P〈0. 05）. Conclusions： Addition and Subtraction Suanzaoren Decoction for MHD patients with depression can reduce the depression degree and improve the sleep quality.
