桃红四物汤对翼状胬肉患者术后免疫功能及血管内皮生长因子、内皮抑素水平的影响

Effect of Taohong Siwu Decoction on Immune Function,Vascular Endothelial Growth Factor and Endostatin Levels for Patients with Pterygium after Surgery

目的:观察桃红四物汤对翼状胬肉患者术后免疫功能及血管内皮生长因子、内皮抑素水平的影响。方法:将我院2014年1月到2017年1月收治的80例翼状胬肉患者纳入研究范围,在常规临床干预路径基础上给予桃红四物汤,另按1∶1比例筛选同期手术治疗并按常规临床路径进行术后干预的80例翼状胬肉患者作为对照组,统计两组症状消失时间及上皮愈合时间,对比两组治疗前后翼状胬肉患者角膜散光、总高阶段成像差情况,比较血清免疫球蛋白A (Ig A)、血清免疫球蛋白M (Ig M)、免疫球蛋白G (Ig G)等免疫功能指标及血管内皮生长因子(VEGF)、内皮抑素(Endostatin)水平,并给予持续随访追踪,统计复发率。结果:观察组术后症状消失时间、上皮愈合时间均较对照组短;两组泪膜破裂时间均延长,角膜散光、总高阶像差、免疫功能指标及VEGF显著下降,Endostatin显著上升,且观察组上述指标下降或上升幅度显著大于对照组;经随访统计,观察组复发率(5. 00%)亦显著低于对照组(15. 00%),上述对比差异均有统计学意义(P<0. 05)。结论:桃红四物汤可改善翼状胬肉患者术后免疫功能及血管内皮生长因子、内皮抑素水平,促进患眼术后恢复,降低复发率。

Objective： To study the effect of Taohong Siwu Decoction on the immune function,vascular endothelial growth factor and endostatin levels for patients with pterygium after surgery. Methods： 80 patients with pterygium who were admitted to our hospital from January 2014 to January 2017 were enrolled in the study,and they were given clinical intervention and treatment with Taohong Siwu Decoction. Another 80 patients with pterygium who were given clinical intervention were selected as the controlled group. The time of symptoms disappearing and epithelium healing time in the two groups were statistically analyzed. The corneal astigmatism and total higher order aberrationswere compared between the two groups before and after the treatment. Serum levels of immune function indexes such as immunoglobulin A（ Ig A）,serum immunoglobulin M（ Ig M） and immunoglobulin G（ Ig G）,vascular endothelial growth factor（ VEGF） and endostatinlevels were also compared between the two groups. All patients were followed up,and the recurrence rate was statistically analyzed. Results： The time of symptoms disappearing and epithelium healing time in the observation group was shorter than that in the controlled group. The breakup time of tear film was prolonged,the corneal astigmatism,total higher-order aberrations,immune function and VEGF were decreased significantly,and endostatin was increased significantly in the two groups,and changes in the observation group were greater than those in the control group. The recurrence rate of the observation group was significantly lower than that of the control group（ 5. 00% vs 15. 00%）（ P〈0. 05）. Conclusion： Taohong Siwu Decoction can improve the immune function and the levels of vascular endothelial growth factor and endostatin in patients with pterygium after surgery. It also can promote the recovery of affected eyes and reduce the recurrence rate.
