脑梗塞针刺治疗早期疗效的MRS评估

An evaluation study on the early therapeutic effect of acupuncture for cerebral infarction using MRS

目的探讨利用磁共振波谱技术(MRS)评估针灸治疗脑梗死早期疗效的价值。方法将28例脑梗塞患者随机分为两组:对照组采用神经内科常规对症及支持治疗,而观察组在常规治疗基础上同时实施针灸治疗。所有病例均在治疗前及治疗结束后2个月行神经功能型缺损评分,并行MRI及MRS检查,测定NAA/Cr和Cho/Cr值,比较治疗前后各指标的变化。结果治疗后两组患者的神经功能型缺损评分均较治疗前显著降低,观察组治疗后评分明显低于对照组(P=0.000)。治疗前NAA/Cr、Cho/Cr值在两组间差异无统计学意义(P> 0.05)。治疗结束后,对照组与观察组NAA/Cr均明显升高(P <0.05),Cho/Cr均明显下降(P <0.05);观察组NAA/Cr升高较对照组明显(P <0.05),而Cho/Cr较对照组下降更为明显(P <0.05)。结论针灸治疗对脑梗塞患者的康复具有一定的疗效,且能被MRS技术所检测。

Objective To study the feasibility of using magnetic resonance spectroscopy （MRS） forevaluating the early therapeutic effect of acupuncture on cerebral infarction. Methods 28 patients underwent cerebral infarction were randomly divided into two groups. The control group received routine symptomatic and supportive treatment in the department of neurology, while the observation group received acupuncture treatment in addition to the routine treatment. All patients received neurologic functional defect score before treatment and 2 months after the end of treatment, and MRI and MRS examinations were also performed. NAA/Cr and Cho/Cr values were evaluated, and the changes of various indicators were compared before and after treatment. Results After treatment, the score of neurological deficit in both groups was significantly lower than that before treatment, and that in the observation group was significantly lower than that in the control group （P = 0.000）. There was no difference in NAA/Cr and Cho/Cr values between the two groups before treatment （P 〉 0.05）. After treatment, NAA/Cr was significantly increased and Cho/Cr was significantly decreased （P 〈 0.05） in the both groups （P 〈 0.05）. NAA/ Cr in the observation group was significantly higher than that in the control group （P 〈 0.05）, while Cho/Cr was significantly lower （P 〈 0.05）. Conclusion Acupuncture has some therapeutic effect on the rehabilitation of cerebral infarction patients, and this effectcan be detected by MRS technology.
