细胞凋亡与Bcl-2蛋白的关系及两者在胃癌发生中的作用

Relationship between apoptosis and Bcl-2 protein and their effects in gastric carcinogenesis

目的探究细胞凋亡与Bcl-2蛋白的关系及两者在胃癌发生中的作用。方法选取我院收集的131例胃镜活检标本及胃癌手术标本进行检测,比较各标本的细胞凋亡指数与Bcl-2蛋白表达情况,并观察两者之间的关系。结果癌前病变标本和胃癌标本的细胞凋亡指数显著高于正常胃黏膜标本,差异显著(P<0.05),胃癌标本的细胞凋亡指数显著低于癌前病变标本,差异显著(P<0.05)。胃癌标本中Bcl-2蛋白阳性表达率显著高于癌前病变标本与正常胃黏膜标本,差异显著(P<0.05)。肠化生标本及胃癌标本的Bcl-2蛋白阴性表达组细胞凋亡指数显著高于Bcl-2蛋白阳性表达组,差异显著(P<0.05)。结论胃黏膜由正常时期进展至肠化生时期,其细胞凋亡指数逐渐提高,但随着病情的进一步发展,细胞凋亡逐渐被抑制;由癌前病变时期至胃癌时期,Bcl-2蛋白的阳性表达率升高,Bcl-2蛋白阳性表达水平的升高与凋亡指数的降低存在密切关联。

Objective To explore the relationship between apoptosis and Bcl -2 protein and their effects in gastriccarcinogenesis. Methods A total of 131 cases of gastroscopic biopsy specimens and gastric cancer surgical specimenscollected in our hospital were examined, then the cell apoptotic indexes and the expression of Bcl-2 protein in specimenswere compared, and the relationship between them were observed. Results The cell apoptotic indexes of the precancerouslesions specimens and gastric cancer specimens were significantly higher than those of the normal gastric mucosaspecimens, the differences were statistically significant （P〈0.05）, but which of gastric cancer specimens were significantlylower than those of the precancerous lesions specimens, the differences were statistically significant （P〈0.05）. The Bcl-2protein expression positive rate in the gastric cancer specimens was significantly higher than those of the precancerouslesions specimens and the normal gastric mucosa specimens, the differences were statistically significant （P〈0.05）. Theapoptosis indexes of the Bcl-2 protein negative expression groups of intestinal metaplasia specimens and gastric cancerspecimens were significantly higher than those of the Bcl -2 protein positive expression groups, the differences werestatistically significant （P〈0.05）. Conclusion The cell apoptotic index gradually increases from the normal period to theintestinal metaplasia period, however it is gradually inhibited with the further development of the disease; from the period of precancerous lesion to gastric cancer, the positive expression of Bcl-2 protein gradually increases, and the increase ofBcl-2 protein positive expression is closely related to the decrease of apoptotic index.
