参附注射液联合PCI手术对急性心肌梗死合并心功能不全患者心功能及预后的作用

Effect of Shenfu injection combined with PCI on improvement of cardiac function and prognosis in patients with acute myocardial infarction complicated with cardiac dysfunction

目的研究参附注射液联合经皮冠状动脉介入(PCI)治疗对急性心肌梗死合并心功能不全患者心功能及预后的改善作用。方法选取2017年1月至2018年1月本院收治的急性心肌梗死合并心功能不全并行急诊PCI的患者76例,随机将其分为观察组和对照组,各38例。对照组采用常规药物治疗,观察组在此基础上静脉滴注参附注射液。比较两组患者的治疗总有效率、治疗前、后心率、心脏指数、心排血量、LVEF水平、血清TNF-α、NO及hs-CRP水平。结果观察组的治疗总有效率为94.74%,显著高于对照组的73.68%(P<0.05)。治疗前,两组患者的心率、心脏指数、心排血量、LVEF水平、血清TNF-α、NO及hs-CRP水平比较,差异均不显著(P>0.05);治疗后,两组患者的心率、TNF-α、hs-CRP水平显著降低,心脏指数、心排血量、LVEF及NO水平均显著升高,且观察组均明显优于对照组,差异均具有统计学意义(P<0.05)。结论参附注射液联合PCI手术能明显改善急性心肌梗死合并心功能不全患者的心功能和临床症状,疗效显著。

Objective To study the effect of Shenfu injection combined with PCI on improvement of cardiac functionand prognosis in patients with acute myocardial infarction complicated with cardiac dysfunction. Methods A total of 76cases of PCI patients with acute myocardial infarction complicated with cardiac dysfunction admitted in our hospitalfrom January 2017 to January 2018 were selected and randomly divided into observation group and control group, with38 cases in each group. The control group was treated with conventional drug therapy, the observation group wastreated with Shenfu injection on this basis. The total effective rate of treatment, heart rates, cardiac indexes, cardiacoutput, the levels of LVEF, serum TNF-琢, NO and hs-CRP were compared between the two groups. Results The totaleffective rate of treatment in the observation group was 94.74%, which was significantly higher than 73.68% in thecontrol group （P〈0.05）. Before treatment, there were no significant differences in heart rates, cardiac indexes, cardiacoutput, the levels of LVEF, serum TNF-琢, NO and hs-CRP between the two groups （P跃0.05）. After treatment, theheart rates, the levels of TNF-琢 and hs-CRP of the two groups significantly decreased, the cardiac indexes, cardiacoutput, the levels of LVEF and NO significantly increased, and those in the observation group were significantly better than the control group, and the difference were statistically differences（P〈0.05）. Conclusion Shenfu injection combinedwith PCI in patients with acute myocardial infarction complicated with cardiac dysfunction can significantly improvepatients＇ cardiac function and clinical symptoms, and it has a significant effect.
