柴芍承气汤联合西医对高血脂性胰腺炎患者炎症反应及血液流变学的影响

Effect of Chaishao Chengqi decoction combined with western medicine on inflammatory reaction and hemorheology in patients with hyperlipidemic pancreatitis

目的探讨柴芍承气汤联合西医对高血脂性胰腺炎患者炎症反应及血液流变学的影响。方法选择2013年12月至2015年12月在我院接受治疗的186例高血脂性胰腺炎患者的病例资料进行回顾性分析,将采用西医常规治疗的患者设为对照组(n=93),采用柴芍承气汤联合西医治疗的患者设为研究组(n=93)。比较两组临床疗效、治疗前、后血清炎症因子、血浆脂联素及血液流变学指标水平。结果研究组治疗总有效率为92.47%,高于对照组的80.65%(P<0.05)。治疗后,两组的IL-6、CRP、TNF-α水平均显著降低,且研究组均明显低于对照组(P<0.05)。治疗后24、72 h、7 d,两组的血浆脂联素水平显著升高,且研究组治疗后同一时间血浆脂联素水平均明显高于对照组(P<0.05)。治疗后,两组的WBV、PV、ESR及HCT水平均显著降低,且研究组均明显低于对照组(P<0.05)。结论柴芍承气汤联合西医对高血脂性胰腺炎疗效显著,可提高血浆脂联素水平,有效降低患者血清炎症因子水平,改善患者血液流变学指标,值得临床推广应用。

Objective To explore the effect of Chaishao Chengqi decoction combined with western medicine oninflammatory reaction and hemorheology in patients with hyperlipidemic pancreatitis. Methods From December 2013 toDecember 2015, the case data of 186 patients with hyperlipidemic pancreatitis treated in our hospital were selected fora retrospective analysis. Among 186 patients, the patients who received western medicine routine treatment were set ascontrol group （n =93）, and the patients who received Chaishao Chengqi decoction combined with western medicinetreatment were set as study group （n =93）. The clinical efficacy, the levels of serum inflammatory factors, plasmaadiponectin and hemorheological indexes before and after treatment were compared between the two groups. Results Thetotal effective rate of treatment in the study group was 92.47%, which was higher than 80.65% in the control group（P〈0.05）. After treatment, the levels of IL-6, CRP and TNF-琢 in the two groups significantly decreased, and those inthe study group were significantly lower than the control group （P〈0.05）. At 24, 72h and 7 days after treatment, thelevels of plasma adiponectin in the two groups significantly increased, and those in the study group were significantlyhigher than the control group at same time after treatment（P〈0.05）. After treatment, the levels of WBV, PV, ESR andHCT in the two groups significantly decreased, and those in the study group were significantly lower than the controlgroup （P〈0.05）. Conclusion Chaishao Chengqi decoction combined with western medicine has a remarkable curative effect on hyperlipidemic pancreatitis, which can improve the level of plasma adiponectin, effectively decrease the levelsof serum inflammatory factors and improve hemorheological indexes. It is worthy of clinical promotion and application.
