2型糖尿病患者血清前蛋白转化酶枯草溶菌素9与胰岛素抵抗指数和C反应蛋白的相关性 被引量:8

Correlations between Level of Serum Proprotein Convertase Subtilisin/kexin Type 9 with Insulin Resistance Index and CRP in T2DM Patients

摘要 目的探究2型糖尿病(T2DM)患者血清前蛋白转化酶枯草溶菌素9(PCSK9)与胰岛素抵抗指数(HOMA-IR)和C反应蛋白(CRP)的相关性。方法对2016年5月—2017年10月就诊的92例T2DM患者的临床资料进行回顾性分析,并将其作为T2DM组。选取同期90例体检健康人群作为对照组。比较2组相关临床指标差异,分析T2DM患者血清PCSK9与各临床指标的相关性,以及T2DM患者血清PCSK9的影响因素。结果 2组间体重指数(BMI)、腰围、舒张压(DBP)、收缩压(SBP)、空腹血糖(FPG)、空腹胰岛素(FINs)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、PCSK9、HOMA-IR和CRP水平比较差异均有统计学意义(P <0. 05,P <0. 01)。T2DM患者血清PCSK9与FPG、FINs、TC、TG、LDL-C、HOMA-IR、CRP均呈正相关关系(P <0. 05)。LDL-C、FPG、HOMA-IR、CRP是T2DM患者血清PCSK9的影响因素(P <0. 05)。结论 T2DM患者的血清PCSK9水平异常升高,且与LDL-C、FPG、HOMA-IR、CRP有密切关系,提示血清PCSK9可能参与了T2DM的发生发展。 Objective To investigate correlations between level of serum proprotein convertase subtilisin/kexin type 9 (PCSK9) with insulin resistance index (HOMA-IR) and C reactive protein (CRP) in patients with type 2 diabetes mellitus (T2DM). Methods Clinical data of 92 patients with T2DM admitted during May 2016 and October 2017 was retrospectively analyzed, and the patients were used as T2DM group, and other 90 healthy person taking medical examination at the same period were selected as control group. Differences of clinical indicators between two groups were compared, and correlations between serum PCSK9 with clinical indicators in T2DM patients and influencing factors of serum PCSK9 in T2DM patients were analyzed. Results There were significant differences in body mass index (BMI) , waist circumference, diastolic pressure (DBP), systolic blood pressure (SBP), fasting blood glucose (FPG), fasting insulin (FINs), total cholesterol ( TC), triglyceride ( TG), high-density lipoprotein cholesterol ( HDL-C), lowdensity lipoprotein cholesterol (LDL-C), PCSK9, HOMA-IR, and CRP levels between two groups (P 〈 0.05, P 〈 0.01 ). There were positive correlations between serum PCSK9 with FPG, FINs, TC, TG, LDL-C, HOMA-IR and CRP in T2DM patients (P 〈 0.05). LDL-C, FPG, HOMA-IR and CRP were influencing factors of serum PCSK9 in patients with T2DM (P 〈 0.05). Conclusion T2DM patients have abnormally elevated serum PCSK9 level, and it is closely related to LDL-C, FPG, HOMA-IR and CRP, which suggestes that serum PCSK9 may be involved in pathogenesy and development of T2DM.
