Relationship between light exposure at night and body mass index in male steel workers

摘要 目的探讨夜光暴露与男性钢铁工人体质指数(BMI)间的关系。方法采用整群抽样方法,以某钢铁生产部门6 299名一线男性钢铁工人为研究对象。通过问卷调查和体格检查收集研究对象相关信息,采用限制性立方样条(RCS)模型分析研究对象工作中夜光暴露时长(LAN)与BMI间的关系;根据RCS模型拟合结果,以LAN为标准将研究对象分为3组,采用多因素Logistic回归模型分析夜光暴露对肥胖的影响。结果钢铁工人肥胖率为19. 3%(1 216/6 299)。调整年龄、婚姻状况、文化程度、家庭人均月收入、吸烟、饮酒、睡眠时间、代谢当量、生活中夜光强度、饮食等混杂因素后,RCS模型分析结果显示,钢铁工人工作中LAN与BMI间存在非线性的剂量-反应关系(整体关联性检验P <0. 05,非线性检验P <0. 05);多因素Logistic回归分析显示,以LAN <1 050组为参照时,LAN为1 050~和2 250~d组钢铁工人LAN与罹患肥胖风险相关,比值比及其95%可信区间分别为1. 19(1. 05~1. 35)和1. 11(1. 11~1. 47)。结论男性钢铁工人夜光暴露与肥胖有关,且其工作中LAN与BMI呈非线性的剂量-反应关系。 Objective To investigate the relationship between light exposure at night( LAN) and body mass index( BMI)in male steel workers. Methods A total of 6 299 first-line male steel workers in a steel production department were included in this study by cluster sampling method. Relevant information was collected through questionnaires and physical examinations. The relationship between accumulated LAN at work and BMI was analyzed using a restricted cubic spline( RCS) model. The study subjects were divided into three groups by the LAN standard according to the results of RCS model fitting. The multi-factor logistic regression model was used to analyze the effects of night light exposure on obesity.Results The obesity rate among steel workers was 19. 3%( 1 216/6 299). After adjusting for age,marital status,educational level,family monthly income,smoking,drinking,sleep time,metabolic parameters,the intensity of night light in life and diet,the results of RCS model analysis showed that there was a non-linear dose-response relationship between accumulated exposure to LAN and BMI( P〈0. 05 for overall association test,P〈0. 05 for nonlinear test); while the results of multiple factors Logistic regression analysis showed that using the group with the LAN 1 050 days as a reference,groups with 1 050 ≤ LAN 2 250 days and LAN ≥2 250 days showed association of exposure to LAN with obesity. The odds ratio and 95% confidence interval was 1. 19( 1. 05-1. 35) and 1. 11( 1. 11-1. 47) respectively.Conclusion Exposure to light at night is associated with obesity in male steel workers. There is a non-linear doseresponse relationship between LAN in their work and BMI.
